Abstract

Occurrence rates of HCV infection notified by different haemodialysis centers in Turkey vary between 20 % and 80 %, and the relationship of HCV positivity with the frequency of transfusion and the duration of haemodialysis is differently interpreted by several groups. The most common recommended approach to avoid from unnecessary and excessive transfusion is to separate the patients from haemodialysis units and wards. In this study, anti-HCV positivity was investigated in the sera of 101 haemodialysis patients every three month periods using a second generation ELISA kit. In the first measurements, anti-HCV was found to be positive in 33 patients (32.6 %). After 15 months, anti-HCV positivity was detected in additional 8 patients (40.6 %). There was no significant correlation between anti-HCV positivity and the number of transfusions (p > 0.05), where as the relation of seropositivity with the number of haemodialysis sessions was highly significant (p < 0.01). A considerable increase in anti-HCV positivity showing no significant relation with the number of blood transfusions suggests that HCV may be transmitted between the haemodialysis patients by a distinct route. To prevent transmission between these patients, the separation of anti-HCV positive patients from haemodialysis units and wards can be recommended.

Türkiye 'de diyaliz merkezlerinde hepatit C virüs (HCV) enfeksiyonunun görülme sıklığı % 20- % 80 oranları arasında değişkenlik göstermekte ve HCV pozitifliğinin transfüzyon sayısı ve diyaliz süresi ile ilişkisi merkezlerce de farklı değerlendirilmektedir. Gereksiz ve fazla transfüzyondan kaçınmanın yanında genelde önerilen uygulama; hastaların makina ve salonunu ayırmaktır. Bu çalışmada 101 diyaliz hastasından 3 aylık periodlar halinde ikinci jenerasyon Elisa kiti kullanarak anti-HCV araştırıldı. Çalışma başlangıcında, 33 hastada (% 32,6) anti-HCV pozitif bulundu. 15 ay sonra 8 hastanın daha serumlarında anti-HCV pozitifliği elde edildi (% 40,6). Anti-HCV pozitifliğinin transfüzyon sayısı ile ilişkisi anlamlı bulunmazken (p> 0.05) diyaliz seans sayısı ile ilişkisi anlamlı bulundu (p < 0.01). Anti-HCV pozitifliğinin ihmal edilmeyecek oranda yükselmesi ve bunun transfüzyon sayısı ile de anlamlı ilişki göstermemesi diyaliz hastalarında kan dışı bulaşın da enfeksiyon yayılanında önemli olduğunu düşündürmektedir. Hastalar arası kontaminasyonun önlenmesinde anti-HCV pozitif hastaların diyaliz makina ve salonlarının ayrılması tavsiye edilir.
