Physiological changes seen in carbohydrate metabolism as a result of perioperative stress

Abstract

The aim of this study was to examine the physiological changes seen in carbohydrate metabolism caused by operative stress and also to compare the effects of two different anesthetic agents in this perspective. In this purpose, we measured the serum insulin, glucose, ACTH and cortisol levels of two groups of patients (n = 15 for each) in pre- and post-operative periods. The patients were divided into two, according to the anesthetic agents (isoflurane and propofol) that were going to be used. We think that, in the isoflurane group serum glucose levels were increased due to the rise in ACTH and cortisol levels in the pre- andpost-operatuar periods. And after 24 hours (B. II) because of the decrease in organic and emotional stress, the fall in ACTH and cortisol levels was followed by a decrease in glucose levels. Accordingly in this period the increase in serum insulin levels might be responsible for the decrease in glucose levels. This profile of changes had not been observed in the propofol group. The reason for this might be the propofol's direct inhibitory effect on cortico-adrenal cells. As a result, the expected rise in cortisol levels due to stress of surgery did not occur. In the B II period probably secondary to the disappearance of the supression effect on adreno-cortical cells, a rise in cortisol levels was seen. Cortisol increases glucose levels probably by supressing the peripheral utilisation of glucose and stimulating gluconeogenesis. In addition, in our work although statistically not significant, the maximal rise found in insulin levels parallel to glucose levels might point out a possible postoperative insulin resistance.

Bu çalışmanın amacı; ameliyat stresinin karbonhidrat metabolizmasında yol açtığı fizyolojik değişimleri incelemek ve iki değişik anestezik ilacı etkileri açısından birbiri ile kıyaslamaktır. Bu amaçla, iki değişik anestezik ajan (izofloran ve propofol) kullanılan ve planlı olarak ameliyata giden iki hasta grubunda (n - 15'er) serum insulin, glikoz, ACTHve kortizol düzeyleri pre- ve post- operatuar dönemlerde ölçülmüştür. İzofloran grubunda serum glikoz düzeylerinin ameliyatın hemen öncesi (A H dönemi) ve sonrasında (B I dönemi) ACTH ve kortizol düzeylerindeki artış nedeniyle yükseldiği düşünülmüştür. Post-operatuar 24 saat sonra (B II dönemi) ise organik ve emosyonel stresin azalmasıyla ACTH ve kortizol düzeylerindeki düşmeyi glikoz düzeylerindeki azalma izlemiştir. Ayrıca, bu dönemde insulin düzeylerindeki belirgin artış da gliseminin azalmasından sorumlu olabilir. İzofloran grubunda, parametreler arasında gözlenen bu uyumlu değişimin propofol grubunda olmadığı saptanmıştır. Bunun nedeni; propofol'ün kortiko-adrenal hücreler üzerindeki direkt inhibisyon yapıcı etkisi ile, ameliyat stresine karşı kortizol yanıtını baskılayıcı etkisi olabilir. B II döneminde olasılıkla propofol'ün adrenokortikol süpresyonunun ortadan kalkmasıyla, buna sekonder bir kortizol artışı olmaktadır. Kortizol ise olasılıkla periferde glikoz kullanımını azaltarak ve glukoneogenezi uyararak kan glikoz değerlerinde artışa yol açmaktadır. Her ne kadar istatistiksel açıdan anlamlı olmasa da, aynı dönemde insulin düzeylerinin de glikoz gibi en yüksek düzeylerde bulunması, operasyon sonrası periferik dokularda fizyolojik bir insulin direncinin geliştiğini gösterebilir.
