The efficacy of single dose antibiotic prophylaxis in transrectal prostate biopsy

Abstract
The efficacy of pre-biopsy single-dose antibiotic prophylaxis on patients undergoing transrectal prostate biopsy were determined in a prospective study. Fifty-seven patients aged between 43 and 84 and who had pre-biopsy negative urine cultures were performed diagnostic transrectal prostate biopsy using 18 Gauge Tru-cut biopsy needle. The patients were randomized to receive orally either single-dose 500 mg ciprofloxacinjust two hours before biopsy (Group 1, n:29) or 500 mg two times a day for 5 days starting the day before biopsy (Group 2, n:28). Urine was obtained for cultures on days 2,5 and 10 post-biopsy. No significant differences were found between groups regarding mean age, accompanying diseases and biopsy number. Two cases in Group 1 had positive urine cultures and one of these patients developed clinical urinary tract infection where in Group 2 only 1 patient had positive urine culture but no clinical infection. As a result we can conclude that single-dose antibiotic prophylaxis for patients undergoing transrectal prostatic biopsy may well be sufficient. .
Özet
Transrektal prostat biyopsisi uygulanan olgularda işlem öncesi tek doz antibiyotik profilaksinin etkinliği prospektif olarak değerlendirildi. Yaşları 43 ile 84 arasında değişen ve biyopsi öncesi idrar kültürlerinde üreme saptanmayan 57 olguya, 18 Gauge "Tru-cut" biyopsi iğnesi ile tanısal amaçlı transrektal prostat biyopsisi uygulandı. Olgulara randomize olarak işlemden 2 saat önce oral tek doz 500 mg (Grup I, n=29) veya işlemden 24 saat önce başlayıp 5 gün süre ile günde 2 kez 500 mg (Grup 2, n=28) siprofloksasin verildi, işlemden sonra 2, 5 ve 10. günlerde idrar kültürü yapıldı. Gruplar arasında yaş, yandaş hastalıklar ve biyopsi sayısı arasında anlamlı farklılık yoktu. Grup l 'de iki olgunun kültüründe üreme ve klinik olarak l olguda üriner enfeksiyon görülürken, grup 2'de yalnız l olguda üreme ve klinik olarak enfeksiyon görülmüştür. Sonuç olarak, transrektal prostat biyopsisi uygulanacak olgularda tek doz antibiyotik profılaksisinin yeterli olabileceği kanısına varılmıştır.
