Evaluation of efficacy of neoadjuvant therapy combining gemcitabine and cisplatin in stage III non-small cell lung cancer

Abstract
The role of new cytotoxic agents like gemcitabine has not yet been proven in the neoadjuvant settings. The aim of the study was to determine the clinical efficacy and toxicity of gemcitabine plus cisplatin as neoadjuvant chemotherapy in patients with stage 111 non-small cell lung cancer. In this study; we administered gemcitabine 1250 mg/m2/day (1+8 day) and cisplatin 70 mglmlday (1 .day) to 14 patients in every 21 days as neoadjuvant chemotherapy who were accepted as having stage 111 non-small cell lung cancer. We examined the efficacy and toxicity after 3 chemotherapy cycles. Patients who had complete response, partial response and stable diseases were sent to surgery. Radiation therapy was planned for the patients who failed to have surgery. In every cycle, hematological toxicities were examined and anemia in 2 patients, leucopenia in 2 patients, febril neutropenia in 1 patient, thrombocytopenia in 4 patients were observed. After chemotherapy, 2 patients were accepted having complete response, 7 patients partial response and 5 patients stable disease. Curative resections were performed in 12 patients. During the follow-up we diagnosed cranial metastases in 3 patients, vertebral metastases in 2 patients, liver metastases in 1 patient. Due to lung infections and progression of primary disease 6 patients died. Median survival of 12 patients undergoing surgery was 34 months. The median survival of the patients who were accepted as unresectable and complete response were 15 months and 30 months, respectively. In conclusion, neoadjuvant gemcitabine and cisplatin combination therapy seems to be a helpful as a treatment choice with high response rates and acceptable toxicity profile in locally advanced non-small cell lung cancers.

Evre 111 küçük hücreli dışı akciğer kanserinde (KHDAK) gemsitabin gibi yeni ajanların neoadjuvan tedavideki rolü henüz netleşmemiştir. Bu çalışmada KHDAK vakalarında neoadjuvan gemsitabin ve sisplatin kombinasyonunun klinik etkinlik ve toksisitesini saptamayı amaçladık. Çalışmaya KHDAK tanısı almış; daha önce tedavi uygulanmamış 14 hasta alındı. Hastalara neoadjuvan tedavi gemsitabin 1250 mglm2lgün (1+8.gün) ve sisplatin 70 mglm2lgün (l.gün) 21 günde bir uygulandı; 3 kemoterapi sonrası bu rejiminin etkinlik ve toksisitesi incelendi. Tanı ve parsiyel yanıt alınan hastalar ile stabil hastalık saptanan vakalar cerrahiye gönderildi; diğerlerine radyoterapi planlandı. Her siklüste hematolojik toksisiteler değerlendirildi ve 2 hastada anemi; 2 hastada lökopeni; 1 hastada febril nötropeni; 4 hastada trom-bositopeni gözlendi. Tedavi sonrası 2 hasta tam yanıt, 7 hasta kısmi yanıt, 5 hasta stabil hastalık olarak değerlendirildi. Hastalardan 12'sine küratif rezeksiyon uygulandı. Takip sırasında 3 hastada kraniyal, 2 hastada vertebra, 1 hastada karaciğer metastazı gelişti. Akciğer enfeksiyonu ve primer hastalık progresyonu nedeniyle vakalardan 6'sı kaybedildi. Öpere olan 12 hastanın ortalama toplam sağkalımı 34 ay olarak saptandı. Anrezektabl olan ile tam yanıtlı olduğu için rezeke edilmeyen vakaların toplam sağkalımı, sırasıyla 15 ay ve 30 ay idi. Sonuç olarak; gemsitabin ve sisplatin rejimi lokal ileri KHDAK hastalarının tedavisinde yüksek yanıt oranları ve kabul edilebilir toksisite profili nedeniyle umut verici bir gelişme olarak görülmektedir.
