Primary posterior capsulotomy with anterior vitrectomy in pediatric cataract surgery

Abstract
Objective: To evaluate the efficacy, results and complica- tions of clear lensectomy and posterior chamber intraocular lens (IOL) implantation in high myopic eyes. Study design: Retrospective clinical study Materials and methods: Twenty eight eyes of 14 patients who were below age 36 months were included in the study. Phacoemulsification surgery performed with general anethesia in all patients. An automated vitreous cutter of phaco machine was used for primary posterior capsulotomy and anterior vitrectomy. None of patients had intraocular lens implantation.Topical treatment was used for 3 months. Postoperative refraction, fundus reflex, area of capsulo- tomy and complications were evaluated. Results: Mean age was 12.6 (2-34) months. Eight of them were female and six were male.The mean follow-up period was 20.16 (12-34) months. Mild corneal stromal edema was detected in 4 of the eyes in postoperative first day. All patients had regular pupil contour and formed anterior chamber. Anterior sinechia and pupil distortion was detect- ed in one patient in the first month and sinechiotomy was made under general anesthesia and in the sixth month con- trol secondary glaucoma has developed in the same patient. In all other patients pupil and fundus examinations were normal and capsulorhexis size was not affecting the vision. Conclusion: Limbal approach lensectomy with posterior capsulotomy and anterior vitrectomy has few complica- tions with good visual and anatomic results in cases with pediatric cataracts.

Amaç: Pediatrik katarakt vakalarında limbal insizyon lensektomi, primer arka kapsülotomi ve ön vitrektomi sonuçlarını değerlendirmek Çalışma düzeni: Retrospektif klinik çalışma Gereç ve yöntem: 36 aydan küçük olan ve genel anestezi altında limbal yaklaşımlı fakoemülsifikasyon cerrahisi uygulanan 14 hastanın 28 gözü çalışmaya alındı. Primer posterior kapsülotomi ve anterior vitrektomi için otomatik vitreus kesici kullanıldı. Hiçbir hastaya göziçi lens implante edilmedi.Topikal tedavi 3 ay kullanıldı. Ameliyat sonrası refraksiyon, fundus reflesi ve komplikasyonlar değerlendirildi. Bulgular: Ortalama yaş 12.6 (2-34 ay) aydı. Sekizi kız, 6'sı erkekti ve izleme süresi ortalama 20.16 (12-34 ay) aydı. Ameliyat sonrası ilkgün 4 gözde geçici korneal stromal ödem saptandı ve bütün gözlerde pupillalar düzenli ve ön kamara normaldi. Birinci ayda bir hastada ön sineşi ve pupil düzensizliği oluştu. Sineşiotomi yapıldı ve 6. ayda sekonder glokom gelişti. Diğer bütün hastalarda pupillalar düzenli ve fundus muayeneleri normaldi. Kapsülotomi genişliklerinin vizyon üzerine olumsuz etkileri yoktu. Sonuç: Pediatrik kataraktlarda limbal yaklaşımlı lensektomi, primer arka kapsülotomi ve ön vitrektomi uygulamasının görsel ve anatomik sonuçları iyi, komplikasyonları azdı.
