Rhinoorbital mycormycosis: A case report

Abstract
Paranasal sinus mucormycosis (PSM) is an invasive, highly aggressive fungal infection that usually occurs in diabetic patients in the setting of ketoacidosis or in immunosup-pressive conditions (such as leukemia, lymphoma, kidney failure). Prompt surgical debridement of all clinically non-viable tissue is required. Systemic antifungal therapy and management of hyperglycemia in diabetic patients are the other cornerstones of treatment. In addition, nowadays hyperbaric oxygen therapy, as an adjunct to surgical debridement and antifungal therapy, is being used. In this article we present a case of rhinoorbilal mucormycosis with diabetic ketoacidosis. We describe the clinical and radio-logic presentation of this case and discuss its etiology, diagnosis, treatment and prognosis.
Özet
Mukormikoz; paranazal sinüslerin nadir görülen, ancak mortalite ve morbiditesi çok yüksek olan bir enfeksiyöz hastalığıdır. Sıklıkla kontrolsüz tip 2 diyabette görülen ketoasidoz. lösemi, lenfoma, kortikosteroid kullanımı ve kronik böbrek yetersizliği gibi durumlarda görülür. Hızlı ve geniş bir cerrahi debridmanın yanı sıra, sistemik antifungal tedavi ve mevcut diabclin kontrol altına alınması tedavideki en önemli noktalardır. Günümüzde bu tedavi modal itelerine ek olarak hiperbarik oksijen tedavisi de kullanılmaya başlanmıştır. Bu yazıda diyabetik keloasidoza bağlı olarak ortaya çıkan bir rinoorbital mukormikoz olgusunu sunarak; hastalığın kliniği, tedavisi ve prognozu ile ilgili özelliklerini gözden geçirdik.
