Evaluation of brucellosis cases

Abstract

One hundred and twenty four cases of brucellosis, hospitalized in the clinic of infectious diseases, Atatürk University Research Hospital were evaluated between October-1985 - August 1996 years. The patients were retrospectively investigated with respect to clinical, epidemiological and laboratory findingis and treatment and complications. Of ihe cases 42 wer women (33,9%) and 82 (64,1%) men with the age range of 14-72 years (average 33,5 years). The history of consuming unpasteuri/ed milk products was present at the 86 (69,4 %) of the cases. Four cases (2 shepherdi, 1 veterinarian and 1 butcher) had history of contact the infected animals. The patients symptom and physical examination findings were as follows: 92,7 % of the cases had fever, 80,6% night sweats, 75,8% osteoarticular manifestations, 42,7% splenomegaly, 21,0% hepatomegaly, 6,4% meningeal irritation findings, 2,4% unilateral epididimorşit and 1,6% endocarditis. The laboratory findings of the patients were as follows: 71,8% of the cases had increased erythrocyte sedimentation rate in 50,0% patients had leucopenia and 10,5% RF - positive, 5,6% patients were blood culture - positive. Recurrence was. seen in 14,5% of cases treatment were applied the combination of doksisiklin 200 mg day for 6 wekks and streptomycin lgday 1M for 3 weeks.

Anabilim dalımızda Ocak 1985-Ağustos 1996 tarihleri arasında yatırılarak izlenen Brusellozlu 124 hasta klinik, epidemiyolojik, laboratuvar bulguları, tedavi ve komplikasyonlar yönünden retrospektif olarak araştırıldı. İncelenen olguların 42'si (%33,9) kadın, 82'si (%64'1) erkekti. Yaş ortalaması 33,5 (14-72) ti. Bulaşmada pastörize edilmemiş süt ürünleri tüketimi öyküsü 86 (%69'4) hastada vardı. İki çoban, bir veteriner ve bir kasaptan hastalıklı hayvanla temas anemnezi alındı. Olguların %92,7'sinde ateş, %80,6'smda aşırı terleme, %75,8'inde romatolojik belirtiler, %42'7'sinde splenomegali, %21.0'inde hepatomegali, %6.4'ünde meninks irritasyon bulguları, %2.4'ünde unilateral epididimorşit ve %1,6'sında endokardit saptandı. Laboratuvar parametlerinden eritrosit sedimentasyon yüksekliği % 71,8, lenfonionositoz % 50,0, CRP pozitifliği %29,0, ALT-AST yükselmesi %21,8, lökopeni %10,5, RF pozitifliği %5,6, hemokültür pozitifliği %4,8, olguda bulundu. Vakaların %14,5'i daha önce bruselloz tanısı konularak tedavi edilen nüks olgularıydı. Tedavide doksisiklin 200 mg gün altı hafta ve Streptomycin lg gün İM üç haftalık tedavi programları uygulandı.
