Posttraumatic interhemispheric subdural hematoma

Abstract

We present the case of a 28-year-old male with interhemispheric subdural hematoma (ISH) secondary to head injury. At admission, Glasgow coma scale score of patient was 10 and he had vomiting, right hemiparesis. Direct craniography did not demonstrate any fracture. An emergency computerized tomography revealed acute subdural hematoma at the right parietal convexity and left half of interhemispheric fissure with moderate midline shift, An emergency evacuation of hematoma was performed by craniectomy. In the operation field, the source of bleeding was not demonstrated. Because of a paucity of reports in the literature concerning this entity, we present a case of traumatic ISH we have recently encountered.

Kafa travmasına bağlı interhemisferik subdural hematom gelişen 28 yaşında bir erkek olguyu sunuyoruz. Başvuru sırasında hastada Glasgow Koma Skala skoru 10 idi; kusma, sağ hemiparezi, saptandı. Kraniografilerde fraktür saptanmadı. Acil yapılan bilgisayarlı tomografide sağ parietal konveksite, interhemisferik fissürün sol yarısında akut subdural hematom ve orta derecede orta hat şifti olduğu belirlendi. Acilen kraniektomi ile hematom boşaltıldı. Operasyon alanında kanama odağı saptanamadı. Literatürde seyrek bildirilmiş bir antite oluşundan ötürü son zamanda karşılaştığımız bu olguyu sunduk.
