Calculation of cut off points in the Turkish version of postpartum depression screening scale

Abstract

The purpose of this research was to determine the Turkish version of Postpartum Depression Screening Scale's (PDSS) cut off points, sensitivity, specificity, positive and negative predictive power for major and minor depression. The research was conducted with 142 women who were determined by a nonrandomized sampling method. The research data were collected between the dates of 01/04/2006 and 22/09/2006 in Aydın province center. The research data were collected using a questionnaire, PDSS and Diagnostic and Statistical Manual of Mental Disorders-fourth edition (DSM-IV) with structured clinical interviews. The PDSS's cut off point for postpartum minor and major depression was determined to be 55. Based on this cut off point the PDSS was found to have an 82% sensitivity, an 85% specificity, a 63% positive predictive power and a 94% negative predictive power in the determination of an individual with minor or major depression. The PDSS's cut off point for postpartum major depression was seen to be 65. Based on this cut off point the PDSS was found to have 91% sensitivity, 89% specificity, 42% positive predictive power, and 99% negative predictive power. According to the study results, these cut off points with the PDSS's sensitivity, specificity, positive and negative predictive power and discriminant function analysis results are at satisfactory level and it can be used in our country.

Araştırmanın amacı, Doğum Sonrası Depresyon Tarama Ölçeği 'nin Türkçe 'de majör ve minör depresyon için kesme noktaları, duyarlılık, özgüllük, pozitif ve negatif yordama gücünü belirlemektir. Araştırmaya olasılıksız örnekleme yöntemi ile 142 kadın alınmıştır. Araştırma verileri 01/04/2006 ve 22/09/2006 tarihleri arasında Aydın il merkezinde toplanmıştır. Araştırmanın verileri soru formu, Doğum Sonrası Depresyon Tarama Ölçeği (DDTÖ) ve Mental bozuklukların Tanısal ve Sayımsal El Kitabı, dördüncü baskı (DSM-IV) ile yapılandırılmış klinik görüşme ile toplanmıştır. DDTÖ 'nin doğum sonrası minör ya da majör depresyon için kesme noktasının 55 olduğu belirlenmiştir. Bu kesme noktasına göre, DDTÖ'nin minör ya da majör depresyonda olan bireyi belirlemedeki duyarlılığının %82, özgüllüğünün %85, pozitif yordama gücünün %63 ve negatif yordama gücünün %94 olduğu saptanmıştır. DDTÖ'nin doğum sonrası majör depresyon için kesme noktasının 65 olduğu görülmüştür. Bu kesme noktasına göre, DDTÖ'nin duyarlılığının %91, özgüllüğünün %89, pozitif yordama gücünün %42 ve negatif yordama gücünün %99 olduğu bulunmuştur. Bu araştırmanın sonuçlarına göre, DDTÖ 'nin duyarlılık, özgüllük, pozitif ve negatif yordama gücü ve ayırma fonksiyon analizi sonuçlarının memnuniyet verici düzeyde olduğu ve ülkemizde kullanılabileceği görülmüştür.
