Gingival hyperplasia in renal allograft recipient receiving cyclosporin: A case report

Abstract
Cyclosporin is a potent immunosuppresive drug is widely used in kidney, liver, heart and other organ transplants, because it is better tolerated than conventional agents such as corticosteroids and cyctotoxic drugs. Despite the overall improvement in graft survival for organ transplants, many adverse effects are associated with this drug. One of the most important adverse effects of cyclosporin is gingival overgrowth. Problems associated with this condition may include poor aesthetics, eating difficulties and fetor oris. This report relates a case of severe gingival hyperplasia in a 25 year old male who had received cyclosporine therapy in conjunction with a kidney transplant. This case was treated by gingivectomy at two appointments, under antibiotic coverage. The oral condition after surgery was satisfactory. The patient is complying with the oral hygiene procedures and his periodontal status is still satisfactory after 8 months of the surgery.
Özet
Siklosporin, kortikosteroidler ve sitotoksikler gibi konvasyonel ilaçlardan daha iyi tolere edildiğinden dolayı böbrek, karaciğer, kalp ve diğer organ transplantasyonunda sıklıkla kullanılan immünosupresif ilaçtır. Siklosporinin organ transplantasyonunda başarıyla kullanılmasına rağmen bir çok yan etkileri vardır. En önemli yan etkilerinden biri gingival hiperplazidir. Bu durum kötü estetik, yeme güçlüğü ve kötü ağız kokusu ile birliktedir. Bu makalede renal transplantasyon yaptırmış olan 25 yaşındaki erkek hastada siklosporin tedavisine bağlı gingival hiperplazi olgusu sunulmuştur. Hastaya antibiyotik baskısı altında 2 seansta gingivektomi operasyonu uygulandı. Cerrahi işlemden sonra oral durum memnuniyet vericiydi. Oral hijyen önerisinde bulunulan ve her ay kontrolü yapılan hastamızın operasyon sonu 8. ayda periodontal dokuların sağlıklı olduğu, nüksün görülmediği gözlendi.
