ENAMEL HYPOPLASIA IN THE PRIMARY DENTITION OF CASE WITH PSORIATIC ERYTHRODERMA AND JUVENILE RHEUMATOID ARTHRITIS

Abstract

Psoriatic Erythroderma is uncommon skin disease in children, in which the entire skin surface has widespread erythema, accompained by edema and desquamation. This case report presents a patient with psoriatic erythroderma and chonic polyarthritis in follow-up since 1,5 years old. During consultation with the pedodontist, he was found to have nonspecific dental hypoplasia. All the necessary extractions and restorations of deciduous teeth were done and decalcified sections prepared from the lower deciduous incisors have shown the enamel hypoplasia. The seven year old patient is regularly followed by dermatologist, rheumatologist and a pedodontist. Supplement of oral function and aesthetic view also supported the patient’s psychologically. This case emphasizes the importance of dental care and during the dental treatment care the drugs which could flare psoriasis and psoriatic arthritis should be avoided

Özet

Psoriatik eritrodermi çocuklarda nadir görülen, tüm deri yüzeyinde yaygın eritemin olduğu ve bu tabloya ödem ve deskuamasyonun da eşlik ettiği bir deri hastalığıdır. Bu olgu da, 1,5 yaşından beri psoriyatik eritrodermi ve juvenil romatoid artrit tanısı alan hasta sunulmuştur. Çocuk dişhekimi tarafından yapılan oral muayenede nonspesifik dişsel hipoplaziler saptanmıştır. Gerekli olan tüm süt dişi çekim ve restorasyonları yapılarak, fizyolojik olarak lükse olmuş alt ön keser dişlerinden hazırlanan dekalsifiye örneklerde süt dişi dentisyonunda mine hipoplazisi bulunduğu sonucuna varılmıştır. Yedi yaşında olan hastamız, dermatoloji uzmanı, çocuk romotoloji uzmanı ve çocuk dişhekimi tarafından takibi yapılmaktadır. Oral fonksiyon ve estetiğin sağlanmasıyla hastada psikolojik olarak da olumlu etki yaratılmıştır. Bu olguda ağız bakımının önemi ve dental tedavi sırasında psoriyatik eritrodermi ve juvenil romatoid artriti alevlendirecek ilaç kullanımından kaçınılmasının önemi vurgulanmaktadır.
