Çocuk suprakondiler humerus kırıklarında cerrahi tedavi sonuçları

Abstract

Purpose: Purpose of this study is to evaluate demographic findings of 220 patients treated with open reduction and stabilization with two cross wires from medial and lateral sides for supracandyalar humerus fracture. The clinical and radiological results are evaluated for the 36 at them available for the lattest follow up. Patients and Methods: The results are evaluated by the criteria of Flynn, modified from Mitchell and Adams. Results: The mean age of the 220 patients at the operation was 7.7 (0.5-15) years. Ninety-three percent extension type and 7% flexion type fracture were detected in a group of 76% boy and 24% girls. In the last examination the mean follow-up of 36 cases, all of whom had been treated by internal fixation with two cross medial and lateral Kirschner wires, was 8.9 (4-20) years. Twenty-four (67%) of the cases were male and 12 (33%) were female with a mean age of 16 (7-33) at the last follow-up. In the evaluation of these patients with Flynn's criteria; 80.5% functionally and 94.5% cosmetically excellent and good results were obtained. Discussion: This study demonstrates that open reduction and stabilization with cross wires in supracondylar humerus fractures has satisfactory results.

Bu çalışma, çocukların deplase suprakondiler humerus kırıklarında açık repozisyon ile medial ve lateralden çapraz konulan iki adet Kirschner teli ile internal tesbit yönteminin uzun takip sonuçlarını değerlendirmek amacıyla gerçekleştirilmiştir. Anabilim Dalımızda 1976-1993 yılları arasında suprakondiler humerus kırığı nedeniyle cerrahi olarak tedavi edilmiş olan 220 çocuğun demografik değerlendirmesi ile kontrol çağrısına cevap vererek son kontrole gelen 36 olgunun klinik ve radyolojik değerlendirmesi yapılmıştır. Bu 220 olgunun operasyon sırasında ortalama yaşı 7.7 (0.5-15) idi. % 76'sı erkek, % 24'ü kız olan hastalarda % 93 ekstansiyon tipi ve % 7 fleksiyon tipi kırık saptanmıştı. Son kontrole gelen ve tümü açık repozisyon ile medial ve lateralden çapraz konulan iki adet Kirschner teli ile internal tesbit yapılmış olan 36 olgunun ortalama takip süresi 8.9 (4-20) yıl idi. Son kontrolde yaş ortalaması 16 (7-33) olan hasta grubunu, 24 erkek (% 67) ve 12 kız (%33) hasta oluşturmaktaydı. Bu hastaların Flynn'in Mitchell ve Adams'tan modifiye ettiği kriterler kullanılarak yapılan değerlendirmesinde; fonksiyonel olarak % 80.5 çok iyi ve iyi, kozmetik olarak da % 94.5 çok iyi ve iyi sonuç elde edilmiştir. Uzun dönem takip sonuçları göz önünde bulundurulduğunda belirtilen yöntem yeterli olarak değerlendirilmiştir.
