Congenital dislocation of the patella (Case report)

Abstract

Purpose of the Study: Review the literature and evaluate our treatment of a congenitally dislocated patella in a 5 months-old infant. Material and Method: A congenitally dislocated patella in a 5 months-old infant with flexion contracture in the left knee was diagnosed with USG. Lateral patellar release and medial retinacular capsuloraphy was managed to relocate the patella in its anatomical position. After the operation extremity was immobilized in cast for 6 weeks period and passive exercises were given after cast removal. Results: In the 3rd year of the follow-up the patella was in its anatomical position. No complications after surgical intervention happened. There was full range of motion on the operated side when compared with the contralateral side. Conclusion: Early detection of the congenitally dislocated patella is important for treating this rarely seen anomaly. Ultrasonic methods are helpful for showing the unossified patella in the dislocatedposition lateral to the femoral condyle. Joint contractures may develope if the problem is lately diagnosed. Lateral retinacular release with medial capsuloraphy may give considerable results in this rarely seen anomaly.

Özet

Konjenital patella çıkığı; patellanın diz hareketleri sırasında femur lateralinde takılı olarak bulunması ile tanımlanan doğumsal bir bozukluktur. Nadir görülen bu patoloji ailesel olabilmekle beraber artrogrypozis multipleks konjenita, Down sendromu, Nail-patella sendromu, Larsen sendromu ve Ellis-Van Crevald sendromu ile beraber olabileceği tarif edilmiştir. Teşhis genellikle gecikmiş olarak patellamn ossifiye olmasından sonra, yaklaşık 3-4 yaşlarında röntgen ile konur. Daha,küçük yaşlarda ultrasonografık yöntemler teşhiste yardımcı olur. Beş aylık bir infantta uyguladığımız teşhis, tanı, tedavi ve 3 yıllık takibimizi bu çalışmamızda bildirmekteyiz.
