Fixation of proximal humeral fractures with modified tension band technique

Abstract
Introduction: Many methods have been proposed so far in treatment of proximal humeral fractures, which created great deal of controversy and confusion. Most proximal humeral fractures are nondisplaced or minimally displaced and can be satisfactorily treated with a sling and early range of motion exercises. The controversy exists when the fractures are significantly displaced. Patients and Methods: Seven patients, 5 females and 2 males, with proximal humeral fracture; which were operated between February 1996 and January 1999 by the same surgeon and with the same technique, were evaluated. The mean age of the patients was 61,1 (22-81). According to Neer's classification, the types of fractures were as follows: 2 two-part fractures, 1 three-part fracture, 2 four-part fractures and 2 fracture-dislocations. All shoulders were approached through deltopectoral incision and after open reposition, the fractures were internally fixated with wire and two cortical screws, using modified tension-band technique. Within two weeks of postoperative, all patients were allowed to begin early active motions. The three-phase rehabilitation protocol, which had been devised by Hudges and Neer, was applied to all patients. Average follow up period was 17 (11-35) months. Pain, daily activity, range of motion and power were rated on a 100 point scale according to Constant-Murley's shoulder rating system. Results: None of the complications that related to displaced proximal humeral fractures such as loosening of correction, avascular necrosis ormyositis ossificans, were seen postoperatively. One patient was considered to have an excellent rating and 6 others were considered to have a good rating. Conclusion: We believe that, tension-band technique, which we modified through using two cortical screws, is an appropriate method of treatment of displaced proximal humeral fractures. It is not too invasive and it provides securely early motion.

Giriş: Humérus üst uç kırıklarının tedavisinde bugüne kadar, büyük tartışmalara yol açan ve kafa karıştıran bir çok yöntem önerilmiştir. Çoğunluğu oluşturan nondeplase veya minimal deplase kırıklar kısa süreli bir immobilizasyon sonrasında erken hareketle başanlı bir şekilde tedavi edilirken, tartışma deplase kırıkların tedavisinde ortaya çıkmaktadır. Hastalar ve Yöntem: Şubat 1996-Ocak 1999 tarihleri arasında, humérus üst uç kınğı nedeniyle aynı cerrah tarafından ve aynı teknik kullanılarak öpere edilen 7 hasta değerlendirildi. 5'i kadın, 2'si erkek olan hastaların ortalama yaşlan 61,1 (22-81) idi. Neer sınıflandırmasına göre kınk tiplerinin dağılımı şu şekilde idi: 2 hastada iki-parçalı, 1hastada üç-parçalı, 2 hastada dört-parçalı kırık ve 2hastada kıtıklı çıkık. Tüm hastalarda, deltopektoral insizyonla girilip kınğın açık repozisyonu sağlandıktan sonra, serklaj teliyle yapılan tansiyon-band yöntemi iki adet kortikal vida kullanımı ile modifiye edilerek internal tespit sağlandı. Tüm hastalarda ameliyat sonrası dönemde iki hafta içersinde erken aktif hareketlere izin verildi. Rehabilitasyon için Hudges ve Neer tarafından önerilen 3 fazlı program uygulandı. Ortalama takip süresi 17 (11-35) ay idi. Hastalar Constant-Murley omuz skorlamasına göre ağrı, günlük yaşam aktivitesi, hareket açıklığı ve kuvvet yönünden, toplam 100 puan üzerinden değerlendirildi. Bulgular: Erken veya geç korreksiyon kaybı, avasküler nekroz ve myositis ossifikans gibi bu tür kırıklara has komplikasyonların hiçbirine rastlanmadı. Sonuçlar l olguda çok iyi, 6 olguda iyi olarak değerlendirildi. Tartışma: Uygulamış olduğumuz tekniğin, deplase humérus üst uç kırıklarının tedavisinde, fazla invazif olmayan ve erken hareket verdirebilecek güvenilirlikte uygun bir yöntem olduğunu düşünmekteyiz.
