Abstract

Introduction: Focus at tuberculosis infection has risen significantly in recent years. Discussion has at surgical techniques in these patients. Material and Methods: Seven patients with primery shoulder tuberculosis were treated in orthopedics clinic of Dicle Üniversty Hospital during 1999-2000 years. Mean age of the patients was 23 years. Five patients were male and two fameles.All patients had open biopsy and chemothreapy with three agents for 12 months of duration. No patients had syneviectomy or primery arthrodesis patients were followed up for 28 months. Results: According to the Wilhinson criteriae 4 patients were accepted excellent, 2 moderate and one bad.Discussion: According to Martini classification we think that only chemotherapy has to be considered for patients in stage I and II, and initially debridman and arhrodesis of necessary for patients in stage III and IV.

Özet

Giriş: Son yıllarda tüberküloz enfeksiyonlarında artış gözlenmesi üzerine, dikkatler tekrar bu hastalığın tedavisine çevrilmiştir. Tartışmalar bu hastalıkta uygulanan cerrahi teknikler üzerinde yoğunlaşmaktadır. Hastalar ve Yöntem: 1995-2000 yılları arasında primer omuz tuberkülozlu 7 hasta tedavi edildi. Beşi erkek, 2'si kadın olan hastaların yaş ortalaması 23 idi. Hastaların tamamına açık biopsi ve 12 ay süreyle üçlü kemoterapi uygulandı. Hiçbir hastaya sinoviektomi ve primer artrodez yapılmadı. Sonuç: Hastalar ortalama 28 ay takip edildiler. Wilkinson kriterlerine göre, hastaların 4'ü mükemmel ve iyi, 2'si orta, l'i ise kötü olarak değerlendirildi. Tartışma: Martini sınıflamasına göre evre I ve H'de sadece kemoterapi, evre III ve IV'te ise başlangıç olarak debritman, daha sonra gerekirse artrodez uygulanması kanısındayız.
