The effect of cold therapy applied to the surface of cast and bandage

Abstract
Objective: To evaluate the effectiveness of cryotherapy in reducing skin temparature under cast and bandage.Patients and Method: Thirty-two healthy subjects were divided into 4 groups, randomly. The sensor of the digital thermometer was secured to the middle of the leg before placement of bandage or cast. Robert Jones bandage, elastic dressing, a below-knee plaster cast and synthetic below-knee cast were applied in group 1, 2, 3 and 4, respectively. Two cold packs were placed around the cast or bandage at the level of sensor, and skin temperatures were recorded for the duration of 90 minutes.Results: The skin temperature of legs decreased significantly with cryotherapy in all groups. The reductions in skin temperature with cryotherapy were more significant in group 3 and 4 than in group 1 and 2.Discussion: The use of cold therapy following extremity trauma and operative procedures is well recognized adjunctive therapy for decades. A plaster or synthetic cast and Robert Jones bandage or elastic dressing do not prevent significant skin temperature lowering by cold packs.

Amaç: Alçı ve bandaj üzerinden yapılan soğuk uygulamanın (Kryoterapi) cilt sıcaklığını düşürmedeki etkinliğini araştırmaktır. Hasta ve Yöntem: 32 denek, rastgele olarak 4 gruba ayrıldı. Bandaj veya alçı yapılmadan önce cilt sıcaklığını dijital olarak ölçen termometrenin algılayıcısı tibia 1/3 orta bölümüne yapıştırıldı. Daha sonra grup 1'deki deneklere diz üzerine kadar çıkan Jones bandajı, grup 2'deki deneklere elastik bandaj, grup 3'e klasik alçı ile kısa bacak alçısı, Grup 4'deki deneklere ise sentetik alçı ile kısa bacak alçısı yapıldı. Algılayıcı seviyesinde alçı veya bandaj üzerine dondurulmuş 2 adet buz paketi koyularak cilt sıcaklığındaki değişim 90 dakika süresince kayıt edildi. Bulgular: 4 grupda da soğuk uygulama ile cilt sıcaklığında belirgin azalma saptandı. Bu azalma istatistiksel olarak anlamlı düzeyde bulundu. Gruplar arası karşılaştırmada soğuk uygulama ile oluşan cilt sıcaklığı azalmasının klasik alçı ve sentetik alçı yapılan deneklerde, elastik bandaj ve Jones bandajı yapılan deneklere göre daha fazla olduğu bulundu. Tartışma: Soğuk uygulama ekstremitelerde travma veya cerrahi müdahale sonrası yıllardır kullanılan bir destek tedavi yöntemidir. Jones bandajı, elastik bandaj, klasik veya sentetik alçı soğuk uygulama sırasında cilt sıcaklığının düşmesini engellemez.
