The Relationship of Adhesion Molecules (ICAM-1, VCAM-1) with TNF-α and Bone Mineral Densitometry in Patients with Ulcerative Colitis during Activation and Remission

Abstract
Objective: The aim of our study is to show whether there is a correlation between adhesion molecules (ICAM-1, VCAM-1) and TNF α and bone mineral density (BMD) in ulcerative colitis (UC) patients during the activation and remission. Materials and Methods: The study included 38 patients with a diagnosis of UC and 38 healthy controls. Serum samples were taken and BMDs were measured with DEXA. Ten patients re- ceived steroid therapy during the activation period. During the remission period, blood samples were taken from the patients and BMD measurements were made with DEXA. Results: We found higher ICAM-1, VCAM-1 and TNF-α levels in activation period patients with UC compared to the remission period. At the same time, we found that patients with UC in both activation and remission periods had lower DEXA values compared to healthy controls, and in addition, patients with UC who received steroid treatment during the activation period had lower DEXA levels compared to patients with UC who did not receive steroids. Conclusion: As a result; TNF-α, ICAM-1 and VCAM-1 levels in UC can guide the clinician in predicting the activation of the disease, and patients with UC should be closely monitored in terms of osteoporosis and fracture risk and their medical treatment should be regulated in this respect. At the same time, we believe that patients with UC who receive steroid therapy during the activation period will be at a higher risk for the development of osteoporosis.
Özet
Amaç: Çalışmamızın amacı, ülseratif kolit (ÜK) hastalarında akti- vasyon ve remisyon sırasında adhezyon molekülleri (ICAM-I, VCAM-I) ile TNF α ve kemik mineral yoğunluğu arasında bir korelasyon olup olmadığını göstermektir. Gereç ve Yöntem: Çalışma, ÜK tanılı 38 hastayı ve 38 sağlıklı kontrol kişiyi içermektedir. Serum örnekleri alındı ve kemik mineral yoğunlukları DEXA ile ölçüldü. Aktivasyon döneminde 10 hasta steroid tedavisi aldı. Remisyon döneminde hastalardan kan örnekleri alınıp, DEXA ile KMY ölçümleri yapıldı. Bulgular: Aktivasyon döneminde UK'li hastalarda remisyon dö- nemine göre daha yüksek ICAM-1, VCAM-1 ve TNF-α düzeyleri bulduk. Aynı zamanda hem aktivasyon hem de remisyon dönem- lerinde ÜK olan hastaların sağlıklı kontrollere göre daha düşük DEXA değerlerine sahip olduğunu ve ayrıca aktivasyon döne- minde steroid tedavisi alan ÜK hastalarının steroid tedavisi al- mayan UK'li hastalara göre daha düşük DEXA düzeylerine sahip olduğunu bulduk. Sonuç: Sonuç olarak; ÜK’de TNF-α, ICAM-1 ve VCAM-1 düzey- lerinin hastalığın aktivasyonunu tahmin etmede klinisyene yol gösterebileceği, ÜK’li hastaların osteoporoz ve kırık riski açısın- dan yakın takip edilmesi ve medikal tedavilerinin bu açıdan da düzenlenmesi gerekmektedir. Aynı zamanda aktivasyon döne- minde steroid tedavisi alan ÜK'li hastaların osteoporoz gelişimi açısından daha fazla risk altında olacağı kanaatindeyiz.
