The evaluation of endometrium by using ultrasonographic, cytologic,histopathologic and microbiologic methods in infertil purebred Arabian mares in vernal transition period.

Abstract
In this study, it was purposed to investigate the condition of the endometrium by using ultrasonographic, cytologic, histopathologic and microbiologic methods in infertile purebred Arabian Mares in vernal transition period. Twelve mares which had failed to regularly produce a foal during last five years were used as material. Guarded, endometrial swabs, clitoral sinus swabs and endometrial biopsy samples were taken for microbiologic, cytologic and histo-patologic examination. The internal genital organs were also ultrasonographically scanned. In ultrasonographic examination of the uterus of twelve mares, six mares had fluid acumulation, cystic uterine structures and purulent contents. Rest of mares, didn't show any abnormal appearance. Positive microbiological results were obtained in four samples and bacteria isolated from all positive swabs(4 samples) were Escherichia coli. Taylorella equigenitalis was not isolated both clitoral sinus swabs and uterine swabs. The accumulation of fluid and debris was determined in the uterus of 3 of 4 mares isolated Escherichia coli, by using ultrasonographic method.However, there was no strong positive correlations between microbiologic and cytologic findings. The distribution of histopathological lesions of twelve endometrial samples classified according to Kenney and Doig (19) were evaluated as category IIA (4 samples ; 33.3 percent), category IIB (5 ; 41.6 percent) and category III (3; 25.1 percent). We observed the weak positive correlation among cytologic and microbiologic findings, ultrasonographic appearance and endometrial biopsy scores. As a result; it is obvious that; ultrasonographical, cytological, histo-pathological, and microbiological examinations have an important role on the monitoring of the endometrial pathological changes that reduce the foalling potantials of the mare. However, it is concluded that a second biopsy should be performed after the treatment to obtain the fertility and the prognosis of the pathological changes.

Bu çalışmada, yaşları 9-19 arasında değişen, infertilite sorunu bulunan ve ilkbahar geçiş dönemindeki- safkan Arap kısraklarında endometriumdaki değişikliklerin ultrasonografik, sitolojik, histopatolojik ve mikrobiyolojik tanı yöntemleri kullanılarak belirlenmesi amaçlandı. Son beş yıldır düzenli yavru vermeyen 12 baş kısrak materyal olarak kullanıldı. Mikrobiyolojik muayeneler için uterustan ve klitoral sinuslardan, sitolojik muayene için uterustan swab örnekleri alındı. Histopatolojik muayeneler için endometriumdan biyopsi örnekleri toplandı. Ayrıca, uterus ultrasonografik tanı yöntemi ile görüntülendi. Fertilite düşüklüğü bulunan Arap kısrakların uteruslarının ultrasonografik görüntülenmesinde 12 kısraktan 6'sında uterusta sekresyon birikimi, kistik odaklar ve purulent içerik belirlenirken diğer 6 kısrağın uterusunda belirgin bir değişkenlik gözlenemedi. Uterustan alınan 12 swab örneğinin sadece 4'ünden Escherichia coli izole edildi. Gerek klitoral sinuslardan gerekse uterustan alınan swab örneklerinde Taylorella equigenitalis izole edilemedi. Escherichia coli izole edilen 4 kısrağın 3 'ünde ultrasonografik tanı yöntemiyle uterusta sekresyon ve kistik odaklara rastlanıldı. Çalışmada, mikrobiyolojik ve sitolojik bulgular arasında etkin bir pozitif ilişki belirlenemedi. Kenney ve Doig'e(19) göre sınıflandırılan 12 endometrial biyopsi örneğindeki histopatolojik lezyonların dağılımı, kategori HA (4 örnek, %33.3 oranında), kategori IIB (5 örnek, %41.6 ora¬nında) ve kategori III (3 örnek, %25.1 oranında) olarak değerlendirildi. Çalışmada, endometrial biyopsi bul¬guları ile mikrobiyolojik, sitolojik ve ultrasonografik bulgular arasında zayıf bir ilişki gözlendi. Sonuç olarak; bu çalışmada kısrakların yavru verim kapasitelerini düşüren endometriumdaki patolojik değişimlerin yorumlanmasında, ultrasonografik, sitolojik, mikrobiyolojik ve histopatolojik muayenelerin etkin bir rol oynadığı saptanmıştır. Ancak fertilite durumunun ve patolojik değişimlerin prognozunun belirlenmesinde, uygulanacak tedavi sonrası ikinci biyopsinin yapılması gerektiği kanısına varılmıştır
