Rheumatoid artritis diagnosed by pleural cytology and open lung biopsy: A case report

Abstract
Rheumatoid arthritis is a systemic autoimmune disease. Pulmonary involvement is the most common extraarticu-lary finding in rheumatoid arthritis. Rheumatiod pulmonary nodule is frequently found in patients who smoke and have subcutaneous nodules. Despite of that pulmonary presentation of rheumatoid arthritis and its taking diagnosis with cytological examination and lung biopsy are rare entities. A 51 year old man was admitted to a hospital with symptoms of cough and joint pain. On his chest radiography pleural effusion in right hemithorax and bilateral multiple lung masses were revealed and he was send to department of thoracic surgery. in radiographic examinations of thorax, pleural effusion in right hemithorax and bilateral multiple soft tissue masses of 3 cm in largest diameter was seen. The nature of the masses couldn'tbe established whet-her they were benign ör malignant. Biochemical tests and cytological examihations of the pleural effusion were adjusted with rheumatoid effusion. According to these findings nodules are thought to be rheumatoid nodules but due to the presence of the progression in nodules and the patient's being a male, smoker and being in the 5th decade, caused the suspicion of malignancy. in this respect right miniposterolateral thoracotomy and wedge resection were performed in order to exclude malignities accompanying to pulmonary rheumatoid nodule. Frozen section examination was shown as benign tumor. Histopathologic examination revealed a necrotic rheumatoid nodule. The long ferm follow up of the patient was uneventfull. Diagnosis of rheumatoid arthritis with cytological examination or lung biopsy is a rare condition. Although pulmonary nodule presence in rheumatoid arthritis as in our case is considered as rheumatoid nodule, lung biopsy is required to exclude malignancies.
Özet
Romatoid artrit sistemik otoimmün bir hastalıktır. Pulmo-ner tutulum romatoid artritin en sık görülen ekstraartiküler bulgusudur. Romatoid pulmoner nodul (RPN) sıklıkla sigara içen ve subkutan nodulu olan seropozitif hastalarda görülür. Ancak romatoid artritin pulmoner prezentasyonu ile ortaya çıkması ve sitoloji veya akciğer biyopsisi ile tanı konması nadir bir durumdur. Öksürük ve eklem ağrısı şikayeti nedeniyle hastaneye başvuran ellibir yaşında erkek hasta, akciğer grafisinde sağda plevral efüzyon ve bilateral akciğerlerde multiple kitle saptanması üzerine kliniğimize sevk edilmiş. Toraksın radyolojik incelemesinde sağda daha belirgin olmak üzere ve en büyüğü yaklaşık 3cm boyutlu malign-benign ayrımı yapılamayan yumuşak doku lezyonları ve sağda plevral efüzyon saptandı. Plevral sıvının biyokimya ve sitolojik incelemeleri romatid efüzyon ile uyumluydu. Bu sonuçlar gözönünde tutulduğunda öncelikli olarak romatoid pul-moner nodul düşünüldü ancak nodüllerin progresyon göstermesi, hastanın sigara içen, 5. dekatta ve erkek hasta olması nedeniyle RPN ile eş zamanlı maligniteyi ekarte etmek için hastaya sağ mini posterolateral torakotomi ve no-düllere wedge rezeksiyon uygulandı. Frozen incelemesi benign rapor edildi. Uzun takip patoloji incelemesinde ise nekrotik romatoid nodul saptandı. Hastanın uzun dönem takibinde komplikasyon gözlenmedi. Romatoid artrit tanısının plevral sıvının sitolojik incelemesi veya açık akciğer biyopsisi ile konulması oldukça nadir bir durumdur. Romatoid artritli olgularda pulmoner nodul varlığında öncelikle romatoid nodul düşünülmekle birlikte malignitenin ekarte edilemediği durumlarda bizim olgumuzda da olduğu gibi akciğer biyopsisi gerekebilir.
