The use of silicone sheet around repaired tendon as a ring to prevent restricting adhesions

Abstract
The Use OfSlıcone SheetAround Repaıred Tendon As A Ring To Prevent Restrıctıng Adhesions Adhesion is the majör problem frequently encountered after the tendon repair, especially of flexor ones, mainly affects the outcome ofsurgery in a negative way. Suture lines and damaged tendon segments are the most frequent sites of dense adhesions especially due to deprivation of paratenon. in this experimental study, we planned to prevent adhesions by using silicone sheet. f ör this reason we wrapped around the repaired tendon segments with silicone as a ring to form paratenon like structure in ten New Zealand White Rabbits. Of these rabbits, the left achilles tendons were usedas the testgroup whereas the rightones were used as the control group. After artificially transecting both tendons and repairing them, we used the silicone sheet encircling the tendon on the left side while the right side remained unwrapped. At the third week and third month after the surgery, histological studies of tendons including macroscopic, light microscopic and motional analysis were made. A functional neosheet formation was observed around the silicone encircled tendon without any nutritional deficit and no adhesions restricting the tendon movements were observed whereas on the right side multiple dense adhesions were detected. So, by this method we prevented adherence of tendon repairs and made a suitable and functional bed forgliding without interfering the tendon nutrition.
Özet
Tendon onarımından sonraki iyileşme döneminde oluşabilecek yapışıklıklar, özellikle fleksör tendon onarımları olmak üzere yapılan cerrahinin sonuçlarını olumsuz yönde etkiler. Paratenon defektinin olduğu bölgeler, sutür hatları ve hasarlı tendon segmentleri yapışıklığa en çok maruz kalan bölgelerdir. Bu deneysel çalışmamızda biz onarım bölgesini silikon yaprak ile sararak bu yapışıklıkları önlenmeyi planladık. Çalışmamızda aynı boy, kilo standartlarında 10 adet Yeni Zelanda beyaz tavşanı kullanıldı. Tavşanların sol asil tendonları test grubu, sağ asil tendonları ise kontrol grubu olarak kullanıldı. Hayvanların her iki asil tendonu düzgün bir şekilde tamamen kesildi ve onarıldı. Sol asil tendonu üzerindeki onarıma sirküler bir şekilde silikon yaprak sarıldı ve cilt kapatıldı. Sağ asil tendonu ise herhangi bir şeyle sarılmadan onarımından sonra üstündeki cilt kapatıldı. 3. hafta sonunda tendonların makroskopik ve mikroskobik histolojik incelemeleri ve hareket analizleri yapıldı. Sonuç olarak silikon ile çevrelenmiş tendon onarımının etrafında fonksiyonel bir kılıf oluştuğu saptandı. Bu kılıf tendonların kayma hareketine izin veriyordu. Ayrıca silikon ile çevrelenmiş bölgede herhangi bir kısıtlayıcı yapışıklığa rastlanmadı. Kontrol grubunda ise onarım bölgesinde ve etrafında çok sayıda yapışıklığa rastlandı.
