Objective versus subjective assesment of the soft tissue profile esthetics

Abstract

The purpose of this study was to compare the objective and subjective evaluations of the soft tissue profile changes in a group of skeletal Class 2 children who were treated by functional appliances. Objective evaluations were done by using lateral cephalograms and Natural Head Position (NHP) photographs, whereas, soft tissue profile outline tracings performed on NHP photographs were used for the subjective assessments. The study comprised of the NHP photographs, lateral cephalometric and hand-wrist radiographs of 8 skeletal Class 2 children who were treated with activator. For the objective assessment, 8 angular measurements were traced and calculated on the NHP photographs and 11 parameters were measured on the lateral cephalometric films. Hand-wrist films were used to determine the growth potential of the patients. Subjective evaluation were done by 10 laypeople and 10 orthodontists who evaluated and scored 16 tracings of before-after treatment soft tissue profile outlines of the 8 patients according to their appearance. T-tests were used to determine before and after treatment differences. Results showing that, the 14 numeric parameters demonstrating significant favourable changes with treatment corresponding well with the results of earlier studies. However, subjective evaluation by orthodontists revealed that the improvement of the profiles after treatment were poor. The amount of improvement was relatively more according to laypeople when compared to orthodontists. We concluded that the "favourable" changes of the numbers derived from cephalometric films and photographic evaluations may not yield much clue whether or not the actual soft tissue profile appearance is pleasing after treatment.

Özet

Bu çalışmanın amacı, fonksiyonel apareylerle tedavi edilmiş iskeletsel Klas 2 malokluzyona sahip çocuklarda meydana gelen yumuşak doku profil değişikliklerinin objektif ve sübjektif olarak değerlendirilmesidir. Objektif değerlendirmeler lateral sefalometrik filmler ve Doğal Baş Pozisyonu'nda (DBP) elde edilen fotoğraflar kullanılarak yapılmıştır. Sübjektif değerlendirmeler için ise fotoğraflar üzerinde çizilen profil siluetlerinden faydalanılmıştır. Çalışma materyalini, aktivatör ile tedavi edilmiş 8 iskeletsel Klas 2 bireyin tedavi başı ve sonunda elde edilen fotoğraf, lateral sefalometrik ve el-bilek filmleri oluşturmuştur. Objektif değerlendirme için, fotoğraflar üzerinde 8 açısal parametre, lateral sefalometrik filmlerde ise 11 parametre ölçülmüştür. El-bilek filmleri kullanılarak bireylerin büyüme potansiyelleri hesaplanmıştır. Sübjektif değerlendirme için ise tedavi başı ve sonu çizilen 16 profil, 10 ortodonti bilgisi olmayan kişiye ve 10 ortodontist'e gösterilmiş ve profillerin görünüm açısından skorlanması istenmiştir. Tedavi başı ve sonu farklılıkların değerlendirilmesinde t-testi kullanılmıştır. Elde edilen sonuçlara göre 14 nümerik parametrede tedaviyle geçmiş çalışmalara benzer önemli ve olumlu değişiklik saptanmıştır. Ancak, sübjektif değerlendirme sonucunda tedaviyle; ortodontistler, ortodonti bilgisi olmayan kişilere göre yumuşak doku profilinde daha az bir iyileşme olduğunu belirtirmişlerdir. Bu çalışmanın sonuçlarına göre, sefalometrik film ve fotoğraflardan elde edilmiş olan olumlu değişiklikler, tedavi sonunda uygun bir yumuşak doku profil görünümünün elde edilip edilemeyeceğine dair bir ipucu vermemektedir.
