The use of buccal sulcus flap in the restoration of residual oronasal fistula (Case report)

Abstract
Various methods of closing maxillary alveolar clefts have been reported. The most frequently applied techniques include the vestibular advancement flap and the buccal sulcus flap. Buccal sulcus flap has some advantages. These advantages include more complete isolation and seal of the graft, improved blood supply due to a continuous flap over the graft, decreased tension as a result of flap release and design, extension of the flap up to the anterior palatal region and absence of periodontal problems during postoperative period due to the lack of pocket incision. In this study, taking these advantages into consideration, buccal sulcus flap was performed and clinical evaluation was carried out following autogenous bone grafting in 4 patients with unilateral residual oronasal fistula
Özet
Maksiller alveolar yarıkları kapamada değişik metodlar rapor edilmiştir. En sık uygulanan teknikler, vestibül ilerletme flebi ve bukkal sulkus flebidir. Bukkal sulkus flebi, bazı avantajlara sahiptir. Bu avantajlar ; greft üzerinde sutur çizgilerinden kaçınarak, greftin tam bir izolasyonu ve örtülmesi, greft üzerindeki kesintisiz flepten dolayı iyi beslenmesi, flep gevşetilmesi ve dizaynının sonucu gerilimin azalması, flebin anterior palatinal bölgeye kadar uzatılabilmesi ve cep insizyonu yapılmadığı için postoperatif dönemde periodontal problemlerin olmamasıdır. Bu çalışmada, yukarıdaki avantajlar göz önünde bulundurularak, unilateral residüel alveol yarıktı 4 hastaya, otojen kemik greftlemesini takiben bukkal sulkus flebi uygulanmış ve klinik değerlendirme yapılmıştır
