Assessment of whole salivary flow rates of dentulous and edentulous geriatric patients

Abstract

Saliva is vitally important for the maintainance of tooth integrity and it facilitates oral functions. For denture wearers, it is also important in the retention of the denture and for the protection of the denture-bearing tissues. The purpose of this study was to determine the changes inflow rates, density and pH of resting and stimulated whole saliva flow rates of the dentulous and edentulous geriatric patients. For the edentulous patients, saliva flow rates after the prosthetic treatment were also measured. It has been suggested that salivary Slow rate decreases with the loss of teeth. Saliva was collected under the clinical conditions between 8.00-10.00 a.m. Measurements of the flow rates of whole saliva were performed at two groups. Group A:dentulous patient, Group B: edentulous patient. Group A measurements were taken as resting and stimulated saliva but Group B were taken at three steps: 1. Resting and stimulated saliva before the prosthetic treatment 2.Immediately after insertion of the complete denture 3.Resting and stimulated saliva after 2-3 months of wearing the complete denture Saliva stimulation was obtained by chewing paraffin wax. Flow rates were calculated as a collected volume/vollection time. It was found that there was a significant difference between the whole saliva flow rates of dentulous and edentulous geriatric patients. Although dentulous patients had higher flow rates than edentulous ones, denture wearing significantly effected the whole saliva rates. As a conclusion, it can be suggested that aging does not necessarily result in degeneration of salivary gland function. So, edentulous patients will have similar saliva flow rate values with the dentulous ones after prosthetic treatment if they don't have any systemic diseases or salivary gland dysfunction.

Özet

Tükürük dişlerin sağlığı ve bütünlüğünde, oral fonksiyonların sağlanmasında büyük önem taşır. Tam protez kullanan bireylerde ise; protezin tutuculuğunu ve protezi taşıyan dokuların korunmasını sağlar. Yapılan çalışmalar, tükürük akış hızının diş kaybıyla azaldığını göstermiştir. Bu çalışmanın da amacı dişli ve dişsiz geriatrik hastalarda stimule edilmiş ve edilmemiş tükürük akış hızı, yoğunluk ve pH değişimlerinin tespitidir. Dişsiz hastalarda, protetik tedaviden sonraki tükürük akış hızıda ölçülmüştür.Tükürük örnekleri, klinik koşullarda sabah saat: 8-10 arası ölçülmüştür. Tüm tükürük akış hızı ölçümleri 2 grupta toplanmıştır. A: dişli bireyler, B: tam dişsiz bireyler Grup A'daki ölçümler stimule edilmiş ve edilmemiş olarak 2 aşamada, 2.grupta ise 3 aşamada toplanmıştır: 1. Protetik tedavi öncesi stimule edilmiş ve edilmemiş tükürük, 2. Tam protezlerin ağıza yerleştirilmesinden hemen sonra, 3. Tam protezlerin 2-3 ay kullanılmasından sonraki stimule edilmiş ve edilmemiş tükürük. Tükürük bezlerinin stimulasyonu hastaların parafin çiğnemesiyle sağlanmıştır. Tükürük akış hızı ise toparlanan tükürük miktarı/toplanan süre olarak hesaplanmıştır. Dişli ve dişsiz bireylerin tüm tükürük akış hızlarında istatistiksel olarak önemli bir farklılık bulunmuştur. Dişli bireylerin akış hızı dişsiz bireylerden fazla olmasına rağmen, dişsiz hastaların protez kullanımı tükürük akış hızlarını önemli oranda arttırmaktadır. Sonuç olarak, yaşlanmayla mutlaka tükürük bezlerinin fonksiyonunda dejenerasyon olmadığını, eğer dişsiz hastaların tükürük bezi disfonksiyonu veya sistemik hastalıkları yoksa protetik tedaviden sonra tükürük akış hızları dişli hastalarla aynı düzeye geldiği tespit edilmiştir.
