Abstract

The vertical dimension lost is considered as an etiological factor for craniomandibular disorders. Management of vertical dimension problems has plagued prostodontist for many years. There are numerous articles and papers on this subject. If vertical dimension could adequately defined in measurable sense , dentists would have the tools to attack the problem where it exits and correct it. The use of modern biomechanical appliances certainly gives dentists the ability to correct these problems. Prostheses for increasing the vertical dimension of occlusion are used in restorative dentistry and orthodontics. The aim of this study is to evaluate the effect of restoring the lost vertical dimension of a TMD patient with prosthetic treatment on the stomatognatic system. A 65 years old woman was attended to our clinic with mild-to-severe recurring episodes of continuous protracted steady dull aching pain diffusely located in the left preauricular area, induced by forcefull or prolonged opening of the mouth and aggravated by jaw use. There is localized tenderness to touch and pressure directly over the lateral pole of the joint. Before treatment, magnetic resonance imaging (MRI) of temporomandibular joint showed internal derangement and disc on the condyle, was anterior position. After 3 months of occlusal splint theraphy, all of the symptoms were diminished so prosthetic treatment of the patient was completed. After 6 months of finishing the prosthetic treatment, MRI showed that the disc came back to its normal position on the condyle.

Özet

Dikey boyut kaybı kraniomandibular bozuklukların etiolojik faktörlerinden biri olarak kabul edilmektedir. Protez uzmanları dikey boyut problemlerini çözmek için yıllardır çalışmalar yapmaktadırlar. Dikey boyut kesin bir ölçüm değeri olarak ifade edilebilse o zaman problemi hemen tespit edip çözmek çok kolay olurdu. Restoratif diş hekimliği ve ortodontide okluzyon dikey boyutunu yükselten protezler sıklıkla kullanılmaktadır. Bu çalışmanın amacı, aşırı dikey boyut kaybı olan bir hastanın protetik tedaviyle kaybolan boyutun kazandırılmasının stomatognatik sisteme etkisini değerlendirmektir. Ağrı şikayeti ile kliniğimize baş vuran 65 yaşında bir kadın hastanın yapılan klinik muayenesi sonunda sol TME bölgesinde yaygın künt bir ağrıya sahip olduğu, uzun süre ağzını açık tuttuğunda ya da çene hareketleri sırasında ağrının artığı, eklemin lateral kutbunun palpasyonunun orta şiddette bir ağrıya neden olduğu saptandı. Tedavi öncesi TME bölgesinden MR incelemesi yapıldı. Hastada internal derangement ve diskin kandilin anteriorunda konumlandığı tespit edildi. 3 aylık okluzal splint tedavisi sonucunda, hastanın tüm semptomları kayboldu ve protetik tedavisi tamamlandı. Protetik tedavinin tamamlanmasından 6 ay sonra, MR görüntülerinde diskin kondilde normal pozisyonuna geldiği tespit edildi.
