Interdisciplinary management of adolescent ectodermal dysplasia patients

Abstract

The aim of this presentation was to present the treatment strategy for a patient with ectodermal dysplasia utilising with removable occlusal splint following fixed prosthesis. A nineteen years old male patient was referred to the clinic for dental esthetic complaints caused by ectodermal dysplasia. Treatment of the patient was planned by the cooperation of orthodontist and prosthodontist. Ectodermal dysplasia patient were treated with fixed bridges because of the number, the shape and the localisation of the favorable abutment teeth. However, for that treatment modality, it was determined that vertical dimension had to be increased at least 2 mm. Resting vertical dimension was determined before increasing the occlusal vertical dimension with full coverage splint. In order to evaluate reactional response for increased vertical dimension, the patient was checked weekly in the first month and thickness modification by grinding was conducted on the occlusal splint. At the end of first month the thickness of the occlusal surfaces of the occlusal splint was 2 mm. The occlusal splint with 2 mm thickness was used by the patient additional two months period. At the second and third month periodic recall visits, the patient had no complaint about clinical stomatognathic symptoms. All maxillary teeth were prepared and metal fused ceramic restorations cemented in situ. The patient rehabilitated with utilising fixed prosthesis with increased vertical dimension showed no clinical stomatognathic symptoms after three years post-rehabilitative six monthly periodic evaluation.

Özet

Bu çalışmanın amacı, ektodermal displazi vakasının hareketli okluzal splint uygulamasını takiben sabit protez ile rehabilite edilmesini sunmaktır. Ondokuz yaşında erkek hasta ektodermal displazi sonucu oluşan dişsel estetik şikayet ile kliniğe başvurdu. Hastanın tedavisi ortodonti ve protez uzmanlarının beraber çalışması şeklinde planlandı. Sayı, şekil ve konumu uygun destek dişlere sahip ektodermal displazi hastasında, sabit protez yapımı uygun görüldü. Ancak, bu uygulama için vertikal boyutun en az 2 mm artırılması gerektiği saptandı. Hastanın okluzal vertikal boyutunun, okluzal yüzeyleri örten splint ile artırılmasından önce istirahat vertikal boyutu saptandı. Artırılmış vertikal boyuta karşı reaksiyonel cevabın değerlendirilmesi için, hasta birinci ay boyunca her hafta kontrol edildi ve okluzal splint aşındırılarak gerekli kalınlık ayarlaması yapıldı. Birinci ayın sonunda okluzal splintin okluzal kalınlığı 2 mm olacak şekilde bitirildi. 2 mm kalınlığındaki okluzal splint, hastaya iki ay daha kullandırıldı. İkinci ve üçüncü ay sonundaki periyodik kontratlarda, hastada hiç bir klinik stomatognatik semptom tesbit edilmedi. Üst çenedeki tüm dişlerin hazırlanmasından sonra, metal destekli seramik restorasyon ile rehabilitasyon bitirildi. Artırılmış vertikal boyutta, sabit protez ile rehabilite edilen hastada, altı aylık periyodik kontroller ile üç yıl sonunda hiç bir klinik stomatognatik semptoma rastlanmadı.
