Cvek's pulpotomy: A case report

Abstract
Pulpotomy is a term, which is used to refer surgically removal of the injured pulp tissue under local anaesthesia. Pulpotomy is an alternative treatment to direct pulp capping or root treatment when the pulp of the young permanent tooth with open apexies was exposed by trauma. When the pulpotomy was performed under optimal conditions, it has been expected that formation of hard tissue barrier at lower level from the capping pulp tissue and continue the development of root. In this case, it has been reported that the succesfull result of a Cvek pulpotomy which had been applied to a 11 numbered tooth of a female patient who is thirteen years-old, and was traumatisedfour years ago. The patient referred to our clinic for fractured composite restorations. In radiographie and clinical examinations it has been observed that the pulpotomy treatment was succesfull, the dentin bridge was formed which was expected and the apical formation was completed. The vitality test was recorded to be positive. Therefore, the endodontic treatment was not needed then only fractured composite restoration was treated. In this case, it has been emphasized that the importance of only performing Cvek pulpotomy could be succesfully without root treatment.
Özet
Pulpotomi, lokal anestezi altında hasarlı pulpa dokusunun cerrahi olarak uzaklaştırılmasıdır. Pulpotomi, kök gelişimi henüz tamamlanmamış, apeksi açık genç bir daimi dişin travmatik pulpa açılımında, direkt pulpa kaplamasına veya kök kanalı tedavisine alternatif bir tedavi yöntemidir. Endikasyon doğru konulduğunda ve pulpotomi kurallara uygun olarak yapıldığında, kaplanan pulpa dokusundan daha aşağı bir seviyede, dentin bariyerinin oluşması ve kök gelişiminin devam etmesi beklenir. Bu olguda 4 yıl önce travmaya maruz kalmış 13 yaşındaki bir kız çocuk hastanın 11 numaralı dişine uygulanmış olan Cvek pulpotomisinin başarılı sonucu bildirilmektedir. Hasta, sadece yapılan kompozit restorasyonlarının düşmesi sebebiyle tekrar gelmiştir. Hastanın klinik ve radyografik muayenesinde, pulpotomi tedavisinin başarılı olduğu, oluşması beklenen dentin köprüsünün teşekkül ettiği, kök pulpası vitalitesinin halen devam ettiği ve apikal formasyonun tamamlandığı gözlenmiştir. Bundan dolayı, hastanın 11 numaralı dişinde kanal tedavisine gerek duyulmamış ve daimi restorasyonu yapılmıştır. Bu vakada, kanal tedavisi yapılmadan, sadece Cvek pulpotomisi uygulamasının da başarılı sonuç verebileceğinin önemi vurgulanmaktadır.
