RESURFACING ARTHROPLASTY FOR HALLUX RIGIDUS: SHORT TERM OUTCOMES

Abstract
INTRODUCTION: Hallux rigidus is a degenerative arthritis of the first metatarsophalangeal joint which progress with pain, formation of dorsal osteophytes and progressive limitation of dorsiflexion. Treatment depends on the patient's age, level of activity, patient expectations and the stage of disease. In recent years, resurface arthroplasty in treatment of advanced stage of hallux rigidus has gained popularity due to advantages such as provide joint movement and protection of metatarsal length. In our study, we aimed to evaluate the functional outcomes and complications of metatarsal surface replacement arthroplasty applied patient due to advanced stages of hallux rigidus.MATERIAL AND METHODS: Metatarsal resurface arthroplasty applied in 7 patients (5 women, 2 men; the mean age 54.0 ± 8.7 years; 5 left side, 2 right side ) due to hallux rigidus between September 2013 - November 2014 were included in the study. Patients were staged radiographically with Hattrup - Johnson hallux rigidus radiological evaluation system. AOFAS scores were used for functional assessment. All complications in patients were recorded. A p value lass than 0.05 was considered to be statistically significant. Wilcoxon paired sample test was used for the comparison of data before and after the surgery.RESULTS: Mean follow-up period was 14.0 months (range, 6-20 months). Six patients have stage 3 and other one has stage 2 hallux rigidus. The higher postoperative mean metatarsophalangeal range of motion (ROM) (50.7 ± 9.3°,) values were obtained compared with preoperative ROM (17.8 ± 10.3°) values p

AMAÇ: Halluks rijidus 1. metatarsofalangeal eklemin dejeneratif artriti olup ağrı, dorsal osteofit oluşumu ve ilerleyici dorsifleksiyon kısıtlılığı ile seyreder. Tedavisi; hastanın yaşı, aktivite düzeyi, hasta beklentisi ve hastalığın evresine bağlıdır. İleri evre halluks rijidus tedavisinde, yüzey değiştirme artroplastisi; eklem hareketine olanak sağlaması, metatars uzunluğunun korunması gibi avantajları nedeniyle son yıllarda popülarite kazanmıştır. Çalışmamızda, ileri evre halluks rijidus nedeniyle metatarsal yüzey değiştirme artroplastisi uygulanmış hastaların fonksiyonel sonuçlarının ve komplikasyonlarının değerlendirilmesi amaçlanmıştır.GEREÇ VE YÖNTEMLER: Eylül 2013- Kasım 2014 tarihleri arasında halluks rijidus nedeniyle metatarsal yüzey değiştirme artroplastisi uygulanan 7 hasta (5 kadın, 2 erkek; ortalama yaş 54.0 ± 8.7 yıl; 5 sol taraf, 2 sağ taraf) çalışmaya dahil edildi.
