THE ANESTHETIC MANANGEMENT FOR EXTRACRANIAL TO INTRACRANIAL (EC-IC) ARTERIAL BYPASS SURGERY

Abstract
The most commonly applied procedure in extracranial to intracranial arterial bypass surgery is anastomosis of the mid cerebral artery with the superficial temporal artery or carotid artery by grafting the radial artery or saphenous vein. This technique can be used in the surgical treatment of giant intracranial aneurysm do not benefit from embolization. However, in addition to the expected risks such as haemorrhage and vasospasm, problems which affect the field of the anaesthetist are hemostasis and anastomosis leakage with heparinisation and anticoagulation used to prevent thrombosis of the peripheral vein which is taken as the graft. The aim of this case report was to evaluate complications which may develop associated with surgery and anaesthesia in the application of the selected anaesthesia during EC-IC bypass surgery.
Özet
Ekstrakranialden intrakraniale arteriyel bypass cerrahisinde en yaygın uygulanan prosedür radial arter veya safen ven grefti alınarak, orta serebral arterin, süperfisiyal temporal arterle veya karotid arterle anastomoz edilmesidir. Embolizasyondan fayda görmeyen dev intrakraniyal anevrizmaların cerrahi tedavisinde bu teknik kullanılabilmektedir. Ancak anevrizma cerrahisinde beklenen kanama, vazospazm gibi risklerin yanında, bu hastalarda greft olarak alınan periferik venin tromboze olmasını önlemek amacıyla kullanılan antikoagülasyon ve heparinizayon ile anastomoz kaçakları ve hemostaz sorunları anestezistin de etki alanındadır. Bu olgu sunumunda Ec-İc bypass ameliyatı sırasında tercih edilen anestezi uygulaması, cerrahi ve anesteziye bağlı gelişebilecek komplikasyonların değerlendirilmesi amaçlanmıştır.
