Akut Gastroenteritli Çocuklarda Rotavirüs ve Enterik Adenovirüs Antijenlerinin Hızlı İmmunokromatografik Yöntemle Araştırılması

Abstract
AbstractInvestigation of Rotavirus and Enteric Adenovirus Antigens in Pediatric Patients with Acut Gastroenteritis by Rapid Immunochromatographic Method Aim: To investigate the precense of rotavirus-adenovirus antigen positivity by employing an immunocromatographic method and to assess both seasonal and age-related distrubution in pediatric patients with acute gastroenteritis. Method: The stool specimens of 781 pediatric patients with the age range of 0-72 months were tested for rotavirus-adenovirus by employing a rapid result-giving commercial kit called VIKIA Rota-Adeno (bioMérieux), which consists of specific monoclonal antibodies and works based on a qualitative immunochromatographic principle, in the Parasitology Laboratory of Medicine School at Mersin University between January 2009 and December 2011. Results: The viral antigens were determined 258 (33%) cases in pediatric patients with acute gastroenteritis. Of these 258 cases, rotavirus was detected in 226 (28.9%), adenovirus in 26 (3.3%), both rotavirus and adenovirus in 6 (0.76%) patients. Ages of the patients detected rotavirus positive (totaly 132 cases, 58.4%) were between 0-24 months. 48 cases of them were in the ages of 24-36 months (21.2%) whereas 22 cases were in 36-48 month (9.7%). On the other hand, the rest were older in age as 11 cases being 48-60 months old (4.8%) and 13 cases being 60-72 months old (5.7%). Most rotavirus antigens positivity rates were determined in November (72 cases, 31.8%), December (58 cases, 25.6%) and January (29 cases, 12.8%). Least rotavirus antigens positivity rate was determined in April (2 cases, 0.8%), May (1 case, 0.4%) and June (2 cases, 0.8%). No seasonal difference in adenovirus distribution had been detected. Conclusion: Rotavirus and adenovirus are the most common pathogens of acute diarrhea in children younger than 2 years old, especially during winter. It should be kept in mind that imunochromatographic methods employing commercial kits allow rapid diagnosis.

Türkçe Özet: Amaç: Akut gastroenteritli çocuklarda rotavirüs-adenovirüs antijen pozitifliğinin immünkromatografik yöntemle araştırılması, mevsimsel ve yaşa bağlı dağılımının incelenmesi amaçlanmıştır. Yöntem: Ocak-2009 ile Aralık-2011 tarihleri arasında Mersin Üniversitesi Tıp Fakültesi Tıbbi Mikrobiyoloji Anabilim Dalı Parazitoloji Laboratuvarı\'na gönderilen, yaşları 0-72 ay arasında değişen akut gastroenteritli 781 çocuk hastanın dışkı örneklerinde rotavirüs-adenovirüs antijenlerinin varlığı, spesifik monoklonal antikorlar içeren ve hızlı sonuç veren kalitatif immünokromatografik prensiple çalışan VIKIA Rota-Adeno (bioMérieux) ticari kiti kullanılarak araştırılmıştır. Bulgular: Gastroenteritli çocukların 258\'inde (%33) viral antijenler saptanmıştır. Bunların 226\'sında (%28.9) rotavirüs, 26\'sında (%3.3) adenovirüs ve altısında (%0.76) rotavirüs-adenovirüs birlikteliği saptanmıştır. Rotavirüs pozitifliğinin yaş gruplarına göre dağılımı sırasıyla, 0-24 ay grubunda 132 olgu (%58.4), 24-36 ay grubunda 48 olgu (%21.2), 36-48 ay grubunda 22 olgu (%9.7), 48-60 ay grubunda 11 olgu (%4.8) ve 60-72 ay grubunda 13 olgu (%5.7) olarak saptanmıştır. Rotavirüs antijen pozitifliği en çok Kasım (72 olgu-%31.8), Aralık (58 olgu-%25.6) ve Ocak (29 olgu-%12.8) aylarında saptanmıştır. Nisan (iki olgu-%0.8), Mayıs (bir olgu-%0.4) ve Haziran (iki olgu-%0.8) aylarında ise en az rotavirüs pozitifliğine rastlanmıştır. Adenovirüs olgularının dağılımında ise mevsimsel bir farklılık görülmemiştir. Sonuç: Özellikle kış aylarında ve iki yaş altı akut gastroenteritli çocuklarda rotavirüs ve adenovirüs en sık görülen viral etkenler olup ticari olarak hazırlanmış immunokromotografik yöntemlerle dışkıdan hızlı tanının sağlanabileceği akılda tutulmalıdır.
