Relation between chronic Helicobacter pylori infection and plasma fibrinogen and lipid

Abstract

In researches on the risk factors of coronary artery disease (CAD), it is indicated that the level of plasma fibrinogen is an independent risk factor and increases the risk of CAD. In the recent years, it has been stated that certain chronic infections may, also, be a risk factor for CAD. Chlamydia pneumoniae and Helicobacter pylori (H. pylori) are regarded as the most significant microorganisms. In some researches, H. pylori is found as a risk factor for CAD; where as, others could not find any positive correlations. In the present study, plasma fibrinogen and lipid levels of patients with dyspeptic complaints were measured and the results were compared between H. pylori positive and negative cases. H. pylori positive and negative cases were determined due to a histological investigation. Patients with diabetes mellitus, hypertension, heart and liver diseases were excluded. Out of 62 cases (mean age: 42.4±13.5) 32 cases (mean age:47.5±l 1.5) were H. pylori positive whereas 30 cases (mean age: 37.1±13.6) were H. pylori negative. The plasma fibrinogen, cholesterol, triglyceride, HDL-cholesterol, LDL-cholesterol and VLDL-cholesterol levels in the H. pylori positive group was; 262.2±72.4 mg/dl., 200.9±46.9 mg/dl., 153.5±80. mg/dl., 45.6±25.5 mg/dl, 127.9±40.4 mg/dl., 31.9±15.7 mg/dl, respectively whereas, the results of the H. pylori negative group was; 251.7±104.2, 203.6±70.2, 154.9±91.1, 49.±10.7, 121.6±45.6, 32.1±17.9, in the same order. There were no significant differences in regard to the plasma fibrinogen and blood lipid values between H. pylori positive and negative groups.

Özet

Koroner arter hastalığı (KAH) risk faktörleri üzerine yapılan çalışmalarda, plazma fibrinojen düzeyinin diğer risk faktörlerinden bağımsız olarak KAH riskini artırdığı gösterilmiştir. Son yıllarda bazı kronik infeksiyonların KAH için risk faktörü olabileceği bildirilmektedir. Clamydia pneumoniae ve Helicobacter pylori (H. pylori) bu enfeksiyonların en çok dikkati çekeni ve araştırılanı olmuştur. H. pylori çalışmaların çoğunda KAH için risk faktörü olarak bulunsa da bazılarında ilişki gösterilememiştir. Bu çalışmada, dispeptik yakınmaları olan, diabet, hipertansiyon, kalp ve karaciğer vb. gibi hastalıkları olmayan, üst endoskopik muayenede gastrit, ülser vb. gibi patolojileri olan, histolojik tetkikte H. pylori pozitif ve negatif saptanan olgularda, plazma fibrinojen ve lipoproteinleri ölçülerek her iki grubun sonuçlan karşılaştırıldı. Toplam 62 olgunun (ortalama yaş: 42.4±13.5) 32'sinde (ortalama yaş: 47.5±11.5) H.pylori pozitif iken 30 olguda (ortalama yaş: 37±13.6) H. pylori negatif idi. H. pylori pozitif gruptaki değerler; fibrinojen: 262.2±72.4 mg/dl, kolesterol: 200.9±46.9 mg/dl, trigliserit: 153.5+80. mg/dl, HDL kolesterol: 45.6±25.5 mg/dl, LDL kolesterol: 127.9±40.4 mg/dl, VLDL kolesterol: 31.9±15.7 mg/dl iken negatif grupta bu değerler sırasıyla; 251.7±104.2, 203.6±70.2, 154.9±91.1, 49±10.7, 121.6±45.6, 32.1±17.9 idi. Sonuçlar karşılaştırıldığında, H. pylori pozitif ve negatif gruplar arasında plazma fibrinojen ve kan lipitleri açısından anlamlı fark bulunmadı.
