Nephrectomies and chronic renal insufficiency secondary to upper urinary system stone disease

Abstract

The aim of the study was to take the notice of the clinicians to the nephrectomy and/or chronic renal insufficiency as a result of delayed diagnosis and management of upper urinary system stone disease. One hundred and ninety-three patients, that were followed with upper urinary system stone disease, between the periods of June 1995 - September 1999, were included in the study. The data of patients were retrospectively analysed in terms of influencing effects of sex, age, site of pathology, previous stone management and complications of upper urinary system stone disease on nephrectomy and chronic renal insufficiency. 31 patients had gone to nephrectomy, 20 (65%) of which were indicated for nephrectomy at the time of diagnosis where as, remaining 11 (35%) had a history of repeated ESWL and surgery. The ratio of nephrectomy had no differences according to the sex and location. Nephrectomy had been performed due to the complications, such as hydronephrosis, hydropyonephrosis, xanthogranulomatous pyelonephritis and chronic pyelonephritic atrophic kidney. Chronic renal insufficiency was appeared in 8 (4%) patients. Borderline chronic renal insufficiency was observed in 10 (5%) and clinical chronic renal insufficiency at the time of diagnosis in 14 (%77) patients. 46 patients (%24) were in paediatric group. The rate of paediatric patients in nephrectomized and chronic renal insufficiency groups were %26 and %39, respectively. Upper urinary system stone disease can lead to nephrectomy and considerable chronic renal insufficiency due to the complications.

Özet

Tanı ve tedavisinde geç kalındığında, üst üriner sistem taş hastalığının neden olduğu nefrektomi ve/veya kronik böbrek yetmezliği (KBY)'ne dikkat çekilmesi amaçlandı. Haziran 1995- Eylül 1999 döneminde üst üriner sistem taş hastalığı tanısı ile kliniğimizde yatırılarak tedavi edilen 193 hasta çalışmaya alındı. Hasta verileri cins, yaş dağılımı, patolojinin lateralizasyonu, geçirilmiş taş tedavisi öyküleri ve taş hastalığı komplikasyonlarının nefrektomi ve/veya KBY gelişimi üzerine etkileri yönünden retrospektif olarak değerlendirildi. 31 hastaya (%16) nefrektomi yapıldığı belirlendi. Nefrektomi yapılan hastaların 20'sinde (%65) tanı konulduğunda nefrektomi endikasyonu vardı. 11 hastada (%35) ise tekrarlayan Extracorporeal Shock Wave Lithotripsy (ESWL) ve/veya operasyon öyküsü belirlendi. Nefrektomi uygulanan hastalarımızda hidronefroz, pyenefroz, kronik pyelonefritik atrofik böbrek ve ksantogranülomatöz pyelonefrit nefrektomi nedenleri olarak belirlendi. 8 vakada (%4) KBY tablosunun yerleştiği ve 10 vakada (%5) ise renal fonksiyonların KBY sınırında olduğu gözlendi. KBY belirlenen hastaların 14'ünde (%77) tanı konulduğunda KBY tablosu vardı. Hastalarımızın 46 (%24)'sı pediatrik yaş grubundaydı. Nefrektomize hastalar arasında pediatrik hasta oranı %26, KBY'liler arasında ise %39 olarak belirlendi. Üst üriner sistem taş hastalığı komplikasyonlara yol açarak nefrektomi zorunluluğu oluşturan ve azımsanmayacak bir oranda KBY'ye neden olabilen bir patolojidir.
