Pseudopelade of brocq: Case report and literature review

Abstract

Pseudopelade of Brocq is the primary cicatricial alopecia of the scalp, and slowly progressive, during which follicular destruction leads to permanent patchy baldness. It was originally described as a clinically non-inflammatory scarring alopecia of unknown origin. On the other hand, some others believe that Pseudopelade of Brocq to represent the atrophic, final "pseudopeladic" stage of several scaring disease, e.g. lichen planoplaris and discoid lupus erythematosus. But, persistence of elastic fibres around the midshaft of the hair follicle was demonstrated in pseudopelade of Brocq, a feature not seen in the other diseases. 30 year-old man presented with a 8-year history of progressive patchy hair loss of the vertex of the scalp. Histologic examination of the scalp biopsy specimen revealed thick bands of connective tissue oriented perpendicular to the overlying epidermis. Their proximity to arrector pilorum muscle suggested that they replaced pilar units; however, diffuse cicatrix was absent. No inflammatory infiltrate was found. We report the case of the pseudopelade of Brocq and reviewed in the aspects of clinical course, histopathology and prognosis.

Özet

Brocq'un pseudopeladı, saçlı derinin primer skatrisyel alopesisi olup progresif seyreder ve kalıcı saç kaybına neden olur. Liken planus veya diskoid lupus eritematozus gibi deriyi tutan bazı hastalıkların son safhasında ortaya çıkan skatrisyel alopesiyi de Brocq'un pseudopeladı olarak adlandıran otörler mevcuttur. Ancak Brocq'un pseudopeladında; skatrize kıl folikülünün etrafında, orta dermişe uyan kısımda elastik fibrillerin korunduğu gösterilmiştir. Bu bulgu, diğer hastalıklara sekunder gelişen alopeside yoktur. Saçlı deride, verteks bölgesinde, 7-8 yıldır giderek yaygınlaşan, multipl alopesik alanlar nedeniyle başvuran 30 yaşındaki erkek hastadan Brocq'un pseudopeladı ön tanısı ile punch biyopsi alındı. Yapılan histolojik incelemede iki noktada, epidermisten subkutan dokuya doğru vertikal olarak seyreden fibröz doku bandı dikkati çekti. Bu skatrize yapıların komşuluğunda arrektör pili kasları normal yapılarını korumuş olarak bulunuyorlardı. Histolojik kesitlerde inflamasyona rastlanmadı. Brocq'un pseudopeladı (primer skatrisyel alopesi) tanısı konulan olgu demonstratif özelliğiyle literatür eşliğinde tartışıldı.
