Postkeratoplasty infectious crystalline keratopathy: A case report and review

Abstract

To review the diagnosis, pathogenesis and treatment of infectious crystalline keratopathy, the definition of a case with infectious crystalline keratopathy following the penetrating keratoplasty for corneal perforation associated with bacterial keratitis has been aimed. A 61-year-old male with infectious crystalline keratopathy was reported. Slit lamp biomicroscopy, anterior segment photography, light microscopic analysis of corneal scraping, cultures for bacteria and fungi were performed. The literature on infectious crystalline keratopathy was reviewed. In slit lamp biomicroscopy, many spiculated crystalline opacities were present in the temporal border of corneal graft extending to its central region. Light microscopic analysis of corneal scraping showed many gram-positive cocci. The cultures grew Streptococcus viridans and Streptococcus faecalis. Despite the cessation of topical corticosteroid treatment and intensive antibiotherapy, only a slow and partial regression in the condition of the case could be achieved. A streptococcal corneal infection may be present as a crystalline keratopathy rather than a typical bacterial keratitis in a postkeratoplasty patient. In this case, the ongoing corticosteroid treatment should be stopped and an intensive treatment with antibiotics to which the causative microorganisms are sensitive should be initiated.

Özet

Bakteriyel keratite bağlı koraeal perforasyon nedeniyle penetran keratoplasti uygulanmış olan bir olguda gelişen İnfeksiyöz Kristalin Keratopati (İKK) nin tanımlanması ve konuyla ilgili güncel bilgilerin derlenmesi amaçlanmıştır. 61 yaşında İKK'li bir erkek olgu sunulmaktadır. Biyomikroskopi, ön segment fotoğrafısi, korneal kazıntı materyalinin ışık mikroskopik analizi, bakteri ve fungus varlığını araştırmak için kültürler yapılmıştır. Biyomikroskopide komeal greftin temporal sınırından santraline doğru yayılım gösteren çok sayıda iğne şeklinde kristalin opasiteler gözlenmiştir. Korneal kazıntı materyalinin ışık mikroskopik analizinde çok miktarda gram-pozitif koklar gösterilmiştir. Kültürde Streptococcus viridans ve Streptoccus faecalis üremiştir. Topikal kortikosteroid tedavisinin kesilmesi ve yoğun bir antibiyotik tedavisine rağmen tabloda ancak yavaş ve kısmi bir gerileme sağlanabilmiştir. Penetran keratoplasti geçirmiş bir olguda streptokokal bir korneal infeksiyon tipik bakteriyel keratit tablosu yanında bir kristalin keratopati şeklinde ortaya çıkabilir. Bu durumda kortikosteroid tedavisi uygulanıyorsa kesilmeli ve etkenin duyarlı olduğu antibiyotiklerle yoğun bir tedavi planlanmalıdır.
