Current Status of Electroconvulsive Therapy: Practices of a Training and Research Hospital in Ankara

Abstract

Aim: In this study based on the medical records of our clinic, we aimed to investigate the therapy frequency and indications and follow-up compliance rates in patients treated with electroconvulsive therapy (ECT) in Turkey. Materials and Methods: We retrospectively reviewed the treatment records and subsequent one-year follow-up data of 105 patients who were hospitalized and underwent ECT at the Psychiatry Clinic of the Ankara Numune Training and Research Hospital in the two-year period between January 2014 and December 2015. Results: ECT was performed in 9.95% of the cases. The most common indication was schizophrenia, followed by bipolar disorder (manic episode). The mean number of ECT sessions was 7.60±2.58. The primary complications due to the treatment were prolonged confusion and cardiovascular system side effects. ECT was mostly preferred due to treatment resistance and refusal of oral intake. During the one-year follow-up, 89.5% of the patients complied with the follow-up protocol and 9.5% were re-hospitalized due to a recurrence. ECT was repeated in only 1.9% of the cases. Discussion and Conclusion: ECT continues to be a safe and effective treatment option. Prospective studies with larger samples can better elucidate the place of ECT among current therapies.

Amaç: Kliniğimiz tıbbi kayıtlarına dayanan bu çalışmada Türkiye’de elektrokonvülsif tedavi (EKT)kullanım sıklığını, endikasyonlarını ve takip süreci hasta uyum oranlarını incelemek amaçlanmıştır.Gereç ve Yöntemler: İki yıllık Ocak 2014—Aralık 2015 döneminde Ankara Numune Eğitim veAraştırma Hastanesi Psikiyatri Kliniği’ne yatırılan ve EKT gören 105 hastanın tedavi ve müteakipbir yıllık takip kayıtları retrospektif olarak incelenmiştir.Bulgular: Hastaların %9,95’ine EKT uygulanmıştı. En yaygın endikasyon şizofreni olup bunu bipolar bozukluk (manik epizot) izlemekteydi. Ortalama EKT seansı sayısı 7,60±2,58 idi. Tedavideen yaygın görülen komplikasyonlar uzamış konfüzyon ve kardiyovasküler sistem yan etkisi idi.EKT en sık tedavi direnci ve oral alım reddi nedenlerinden ötürü tercih edilmişti. Bir yıllık takipsürecinde hastaların %89,5’i takip protokolüne uymuş olup %9,5’i nüks nedeniyle yeniden hastaneye yatırılmıştı. Mükerrer EKT hastaların sadece %1,9’unda uygulanmıştı.Tartışma ve Sonuç: EKT güvenli ve etkili bir tedavi seçeneği olmayı sürdürmektedir. EKT’ningüncel tedaviler arasındaki yerinin daha iyi anlaşılabilmesi için geniş örneklemli, prospektif çalışmalar yapılması yararlı olabilir.
