Accompanying embolectomy and closed fasciotomy in the same session for the delayed arterial thromboembolic occlusions in lower extremity

Abstract
Objective: We aimed to compare the postoperative complications in patient with acute lower extremity arterial occlusion who admitted in the late period in whom accompanying embolectomy and fasciotomy in the same session and fasciotomy following embolectomy were applied, because of development of compartment syndrome. Methods: A total of 36 patients (13 female, 23 male) with acute lower extremity arterial occlusion, who admitted to our clinic at least 12 hours after onset of symptoms were enrolled to the study and investigated retrospectively. While embolectomy was performed in all cases, the cases in which mini fasciotomy was performed after development of compartment syndrome constitute the group 1 (n=21) and the ones in whom accompanying embolectomy and closed mini fasciotomy in the same session constitute the group 2 (n=15), groups were formed randomly. Amaç: Alt ekstremite tromboembolik arteriyel tıkanma tanısı ile geç dönemde başvuran, embolektomi ile aynı seansta henüz kompartman sendromu gelişmeden fasiyotomi yapılan hasta grubu ile embolektomi sonrası kompartman sendromu gelişmesi sonrasında fasiyotomi yapılan hasta grubundaki postoperatif komplikasyonları karşılaştırmayı amaçladık. Yöntemler: Semptomların başlamasıyla hastaneye başvurma zamanı arasındaki süre 12 saatten uzun olan alt ekstremite akut tromboembolik arteriyel tıkanma tanısıyla kliniğimize başvuran ardışık 36 hasta (13 kadın, 23 erkek) çalışmaya dahil edildi ve retrospektif olarak incelendi. Tüm olgulara embolektomi yapıldı. Embolektomi yapıldıktan sonraki süreçte kompartman sendromu gelişmesi nedeniyle kapalı mini fasiyotomi yapılan hastalar grup 1’i (n=21), kompartman sendromu gelişmeden embolektomi ile aynı seansta kapalı mini fasiyotomi yapılan hastalar grup 2’yi (n=15) oluşturdu, gruplar rastgele oluşturuldu. Gruplar, klinik takiplerinde gelişen amputasyon, renal yetmezlik, alt ekstremitede hipoestezi, parmak uçlarında parezi gibi morbid faktörler yönünden karşılaştırıldı.
