Abstract

Objective: This study aimed to determine the most common bleeding type, he location of bleeding, the type of trauma and the presence of fracture in patients admitted to the emergency department of our hospital following head trauma. Material and Method: This study evaluated patient information from the Enlil-HIS data system of our hospital and the case files of patients admitted o the emergency department following head trauma. The study included 47 major head trauma patients, of whom 15 were female (68.1%) and 32 were male (31.9%), with either fracture or bleeding or both in the cranium. Results: The most common cause of major trauma was traffic accidents and the second most common was falling. It was determined that major trauma was more common in males than in females; 14.9% of cases resulted in death. The majority of deaths occurred in patients over the age of 65 (42.8%). The occurence of head trauma with fracture and concomitant bleeding was significantly high (38.3%). There was no statistically significant difference in the occurrence of hemorrhage with or without fracture (p>0.05). The most common locations of bleeding were the frontal, frontoparietal and temporal regions, and the maority of bleeds were subdural (25.5%) and subarachnoidal (23.4%). Conclusion: The possibility of brain hemorrhage in cases of head trauma without fracture should be considered upon admission to the emergency department. (JAEM 2012; 11: 85-8)

Özet

Amaç: Bu çalışmada hastanemiz acil servisine kafa travması nedeni ile başvuan, kranium kırığı veya beyin kanaması olan hastalarda kanama kırık ilişkisinin incelenmesi amaçlandı. Gereç ve Yöntemler: Çalışmamız; acil servise kafa travması nedeniyle başvuan hastaların, hasta dosyaları ve hastanemiz Enlil-HIS veri sisteminden elde edilen hasta bilgileri kullanılarak yapıldı. Çalışmaya kafatasında kırık, kanama ya da her ikisi birden bulunan ağır travmalı 15i kadın (%68,1) ve 32si erkek %31,9) olmak üzere toplam 47 olgu dahil edildi. Bulgular: Çalışmamız sonucunda ağır kafa travmalarının en sık nedeni traik kazaları, daha sonra da düşmeler olarak bulundu. Ağır kafa travmalarının erkeklerde kadınlara oranla daha fazla olduğu ve %14,9unun ölümle sonuçandığı tespit edildi. Ölenlerin çoğunluğunu (%42,8) 65 yaş üzeri olgular oluşuruyordu. Kafa travmalarında kırıkla birlikte kanama görülme oranı yüksekti %38,3). Kırıkla birlikte kanama görülmesi ile kırık olmadan kanama görülmesi arasında istatistiksel olarak anlamlılık mevcut değildi (p>0,05). Kanamaların çoğunluğu frontal, frontoparyetal ve temporal bölgelerde görülmekle birlikte bunların çoğu subdural (%25,5) ve subaraknoid (%23,4) kanama idi. Sonuç: Acil servise başvuran kafatasında kırık olmayan travma hastalarında beyin kanaması olma ihtimalinin kırık olanlar kadar yüksek olabileceği göz önünde bulundurulmalıdır. (JAEM 2012; 11: 85-8)
