112 ile acil tıp servisine getirilen erişkin hastaların analizi

Abstract
Objective: In this study we evaluated the convenience of the transports and the prehospital interventions of the cases admitted to a third level hospital with the 112 ambulance service together with their demographic features. Material and Methods: All of the adult patients admitted to the hospital with the 112 ambulance service between January 2009, 1 and January 2010, 31 were included in the study. Patients were evaluated with the 112 ambulance patient register form data collected from the archives of the hospital and the computer recordings. Variables were sex, transport duration, admission time, place of event, reason of the transport, prehospital interventions of the ambulance team, initial diagnosis, disposition of the patint recorded in the emergency department. Results: The ambulance use rate of the patients admitted to our hospital was 0.54%. Of the patients 53.5% were male and 46.5% were female. The admission time from the scene of event was 20-29 minutes. 49% ofpatients were transported enrolled from the home in 49.3%. Of the 75.1% patients transported for diagnosis and treatment, 17.6% had been sent for consulta- tion and 7.3% dispatched from an other hospital. 82.6% of the dispatched pa- tients were admitted without approval. Forty precentage of the dead patients admitted to the hospital by ambulance were not intubated. Thirty-eight percentage of the intubated patients were reintubated in the emergency department after admission. Of the patients transported by the 112 ambulance service, 74% were discharged and 24.1% were hospitalised. Conclusion: The interventions carried out on the patients admitted to the emergency department by ambulance are not sufficient. It is thought that the education programmes are concentrated on inservice and the emergency medical services organisation should be developed. (JAEM 2013; 12: 150-4)

Amaç: Bu çalışmada 3. basamak bir hastaneye 112 ambulans hizmeti ile getirilen olguların demografik özellikleri ile beraber hastane öncesi yapılan müdahalelerin ve sevk işlemlerinin uygunluğunu değerlendirdik. Gereç ve Yöntemler: Geriye dönük yapılan bu çalışmaya 1 Aralık 2009 ile 31 Aralık 2010 tarihleri arasında 112 ile getirilen tüm erişkin hastalar alındı. Hastalar, hastanemiz arşivinden alınan 112 ambulans hasta kayıt formları ve hastane bilgi işlem bilgisayar kayıtları incelenerek değerlendirildi. Araştırmanın değişkenleri; kayıtlarda var olan cinsiyet, hastaneye varış süresi, geliş zamanları, olayın ger- çekleştiği yer, ne için getirildiği, ambulans ekibinin hastane öncesi acil bakım müdahalesi, ön tanıları ve acil serviste sonlanım durumu olarak kaydedildi. Bulgular: Hastanemize gelen hastaların yıllık ambulans kullanma oranı %0,54 idi. Bu olguların %53,5i erkek, %46,5i kadındı. Hastaların %45,5i olay yerinden hastaneye 20-29 dakika arasında getirilmişti. Hastalar en sık %49,3 ile evden alınmıştı. Hastaların %75,1inin tanı ve tedavi için, %17,6sının konsültasyon için ve %7,3ünün ise sevk sebebiyle hastanemize getirildiği belirlendi. Sevk edilen hastaların %82,6sının ise onamsız olarak getirildiği saptandı. Ambulans ile ölü duhul olarak getirilen vakaların %40ı hiç entübe edilmemişti. Entübe edilenlerin %38i ise acil serviste yeniden entübe edildiği saptandı. 112 ambulansı ile getirilen hastaların %74,9u acil servisten taburcu edilirken, %24,1ine hastaneye yatış verilmişti. Sonuç: Ambulans ile acil servise getirilen hastalara yapılan müdahaleler yeterli olmamaktadır. Bu durum ambulanslarda çalışan sağlık personellerinin hizmet içi eğitimine daha fazla ağırlık verilmesi ve acil sağlık hizmetlerinin organizasyo- nunun geliştirilmesi gerektiğini düşündürmektedir.
