Arterialization of venous system for revascularization of ischemic leg (Experimental study)

Abstract
Background: In this study the effectiveness of staged arteriovenous reversal technique in a sheep model with ischemic extremity was tested. Method: Merino type sheep were divided into three groups each containing five sheep, and three different surgical techniques were applied. In-group one, distal of artery was ligatured immediately after right femoral arteriovenous anastomosis. In the second group following right femoral arteriovenous anastomosis, on the 11th postoperative day and arteriovenous reversal was made. Proximal of the vein was ligatured in both groups postoperatively on the 11th day. In-group three, only left femoral femoral artery was ligatured at the proximal. One subject randomly chosen from each group underwent digital substract angiography (DSA) and 99mTc macro aggregated albumin (MAA) syntigraphy. Again these subjects were pathologically evaluated on the biopsy samples taken from adductor and semimembranous muscles of ischemic extremity. Besides, the thigh thickness differences, tension arterial (TA), TA levels of extremities compared with the other extremity and whether necrosis existed in distal extremity were evaluated postoperatively on the day of operation, 10th day, 20th day, 30th day and 40th day. ATP evaluation was assessed in the tissue samples taken on the 40th day preoperatively and postoperatively. Clinical findings were more favorable in group 2. Most favorable arterial pressure, syntigraphic appearance and angiographic arterialization were found in group 2. In histopathologic evaluation, ischemic injury was found to be moderate in group 1, mild in group 2 and severe in group 3. Biochemical analysis showed highest level of ATP in group 2. Result: In conclusion, gradedly applied staged arteriovenous reversal technique was thought to be applicable on patients with ischemic extremity, which cannot be treated with other surgical reconstruction treatments.
Özet
Amaç: Bir ekstremitesi iskemili koyun modelinde, kademeli arteriovenöz reversal tekniğinin etkinliği bu çalışmada test edildi. Metot: Çalışma; 5'erli 3 gruba ayrılan 15 merinos cinsi koyunda 3 farklı operasyon tekniği uygulanarak gerçekleştirildi. 1. gruptaki deneklerin sağ femoral arteriovenöz side to side anastomozdan sonra arter distali hemen, 2. gruptaki deneklerin ise anastomozdan sonra 11. gün arter distali ligatüre edildi. Her iki gruptaki deneklerde ven proksimali anastomozdan sonraki 11. gün ligatüre edildi. 3. gruptaki deneklerin ise anastomoz yapılmadan sol femoral arterleri ligatüre edildi. Her gruptan rastgele seçilen birer hayvanda dijital substract anjiografi (DSA) ve 99mTc makro agregated albümin (MAA) sintigrafisi çekildi ve iskemik ekstremitenin addüktör ve semimembranöz adalelerinden alınan biyopsi örneklerinde iskemik hasarın histopatolojik değerlendirmesi yapıldı. Ayrıca her hayvanın uyluk çevre farkları, tansiyon arterial (TA), sağ/sol ekstremite TA oranları ve distal ekstremite nekrozunun olup olmadığı preoperatif, postoperatif ilk gün, 10.gün, 20.gün, 30.gün ve 40.günlerde değerlendirildi. Yine her hayvandan preoperatif ve postoperatif 40.gün alınan doku örneklerinde ATP tayini yapıldı. Klinik bulgular, 2. grup lehine iyi idi. En iyi TA, sintigrafik tutulum ve anjiografik arteriyelizasyon 2. grupta tespit edildi. Histopatolojik değerlendirmede 1. grupta orta derecede, 2. grupta hafif, 3. grupta belirgin iskemik hasar tespit edildi. Biyokimyasal analiz 2. grupta en yüksek ATP değerlerini gösterdi. Sonuç: Elde edilen verilere dayanarak; kademeli olarak yapılan staged arteriovenöz reversal tekniğinin diğer cerrahi rekonstrüksiyon tedavileri ile sonuç alınamayan iskemik ekstremiteli vakalarda uygulanabilir bir girişim olabileceği kanaatine vardık.
