Color doppler ultrasonography guided compression therapy of iatrogenic postcatheterization pseudoaneurysms of femoral and brachial arteries

Abstract

Background: To evaluate the effectiveness of color Doppler ultrasound guided compression therapy of postcatheterization pseudoaneurysms and the factors that may effect the success. Methods: Twenty-two pseudoaneurysms were included in the study. Direct compession was applied by the lineer probe on the neck of lesions in a manner not obstmating the main artery flow. The pseudoaneurysms thrombosed after compression formed the successful group and the lesions didn’t thrombose formed the unsuccessful one. To investigate the factors that may effect the results (age, sex, usage of anticoagulants and/or antiplatelets, origin, age and volume of lesions, dimensions of neck, flow speed in neck, and total compression time) were recorded. Results: Fifteen pseudoaneurysms originated from main femoral artery, four from superficial femoral artery and three from brakial artery. The two cases that could not tolerate pain were sent to surgery. Eighteen cases totaly thrombosed after compression. In two cases, it was unsuccessful. No complications occurred. The compression therapy of femoral and brachial postcatheterization pseudoaneurysms was successful 90%. The quantitative factors that may effect the results could not be evaluated as there were not enough cases in one of the groups. The qualitative factors did not have any significant effect on the results. However, the relation between the usage of anticoagulants and the results was nearer to the statistical border than the other factors. Conclusions: It was shown that the color Doppler guided compression therapy of pseudoaneurysms was effective and safe, so it could reduce the need for surgery.

Özet

Amaç: Arteriyel kateterizasyona sekonder psödoanevrizmaların renkli Doppler ultrasonografi eşliğinde kompresyonla tedavisinde etkinliğin ve başarıyı etkileyen faktörlerin belirlenmesi. Materyal ve Metod: Kateterizasyon sonrası girişim yerinde psödoanevrizma tesbit edilen 22 olgu çalışma kapsamına alındı. Renkli Doppler ultrasonografi cihazının lineer probu anevrizma boynundaki akımı kesecek, ancak ana arterdeki akımı kesmeyecek şekilde yerleştirilerek kompresyon yapıldı. Ağrıyı tolere edemeyen olgularda cerrahi tedavi uygulandı. Kompresyon tedavisi sonucunda lezyonun tam olarak tromboze olduğu olgular başarılı grubu, olmadığı olgular başarısız grubu oluşturdu. Toplam 30 dak’lık tedavi sonrasında, içerisinde hiç trombüs oluşmamış olgularda tedavi başarısız kabul edildi. Sonucu etkileyen faktörleri araştırmak amacıyla olguların yaşları, cinsiyetleri, antikoagülan ve/veya antiagregan ilaç kullanıp kullanmadıkları, lezyonun kaynaklandığı arter, tedavi öncesinde trombüs içerip içermediği, yaşı, hacmi, boyun uzunluğu, boyun kalınlığı ve boyunda akım hızı belirlendi. Toplam tedavi süreleri kaydedildi. Bulgular: Toplam 22 psödoanevrizmanın 15’i ana femoral arter, dördü yüzeyel femoral arter ve üçü brakiyal arter kaynaklıydı. İki olgu ağrıyı tolere edemediğinden cerrahiye sevk edildi. Tedavi uygulanan 20 olgudan 18’inde tam tromboz sağlanırken, iki olguda tedavi başarızsız oldu. Tedavi sırasında ve sonrasında herhangi bir komplikasyon gelişmedi. Lezyonların kompresyonla tedavisi %90 oranında başarılı oldu. Sonucu etkileyebilecek parametrelerden niceliksel olanlar gruplardan birinde yeterli sayıda olgu bulunmadığından istatistiksel olarak değerlendirilemedi. Niteliksel faktörlerden hiçbiri sonucu anlamlı olarak etkilemiyordu. Antikoagülan varlığı ile sonuç arasındaki ilişki diğer parametrelere göre istatistiksel olarak anlamlılık sınırına daha yakın bulunmakla beraber anlamlılık farklı değildi. Sonuç: Kateterizasyon sonucu gelişmiş psödoanevrizmaların renkli Doppler ultrasonografi eşliğinde kompresyonla tedavisinin güvenli ve etkili bir tedavi yöntemi olduğu, cerrahi müdahale gereksinimini azaltabileceği kanaatindeyiz.
