Analysis of 56 patients who underwent tracheal surgery

Abstract
Background: Tracheal surgery developed later than the other fields of thoracic surgery due to its relatively uncommon occurrence and also due to currently developed techniques regarding the establishment of safe airway and anesthetic support which are the prerequisites for tracheal surgery. Herein, we present our experience in tracheal surgery.Methods: A total of 56 patients (35 males 21 females; mean age 39 years; range 7 to 64 years) who underwent tracheal surgery between January 1976 and January 2006 were retrospectively evaluated. Patients were analyzed according to age, gender, primary disease, symptoms, surgical procedures and follow-up. Of the 56 patients, 21 had primary tracheal tumors, six had benign tracheal tumors, 24 had postintubation tracheal stenosis and five had posttraumatic tracheal injury. All of the patients were evaluated with rigid bronchoscopy after initial diagnosis.Results: Dyspnea, tachypnea and cough were the most common symptoms of presentation. The surgical procedures performed were; primary tracheal repair, tracheal resection and end-to-end anastomosis, wedge resection and primary repair, carino-broncho-tracheal resection and reconstruction, and inferior tracheostomy. Postoperative morbidity and mortality rates were 8.9% and 1.7% respectively.Conclusion: Although tracheal surgery is challenging because of anatomic and technical factors, it is important that, it can be curative in benign lesions and can give satisfactory results with additional radiation therapy. Results of tracheal surgery performed by an experienced surgical team in appropriate centers is rather successful.

Amaç: Trakea hastalıklarının nispeten az görülmesi ve ameliyat için özellikle gerekli olan güvenli hava yolu ve anestezi desteğinin son dönemlerde sağlanması gibi nedenlerle, trakea cerrahisinin gelişimi, göğüs cerrahisinin diğer alanlarına göre daha geç olmuştur. Bu çalışmamızda trakea cerrahisi ile ilgili tecrübelerimizi sunduk.Çalışma planı: Ocak 1976 - Ocak 2006 tarihleri arasında trakea cerrahisi uygulanan 56 olgu (35 erkek, 21 kadın; ort. yaş 39; dağılım 7-64) retrospektif olarak incelendi. Hastalar yaş, cinsiyet, primer hastalıklar, semptomlar, cerrahi yaklaşımlar ve takip yönünden analiz edildi. Olgulardan 21'i trakeanın malign tümörü, altısı trakeanın benign tümörü, 24'ü post-entübasyon trakea stenozu, beşi travma sonrası gelişen trakea yaralanmasıydı. İlk tanıları takiben tüm olgular rijit bronkoskopi ile değerlendirildi.Sonuçlar: En sık başvuru semptomları; dispne, takipne ve öksürüktü. Hastalara uygulanan ameliyatlar; primer trakea tamiri, trakea rezeksiyonu ve uç uca anastomoz, trakeaya wedge rezeksiyon ve primer tamir, karino-bronko-trakeal rezeksiyon ve rekonstrüksiyon, ve inferior trakeostomiydi. Ameliyat sonrası morbidite oranı %8.9 ve mortalite oranı %1.7'ydi.Tartışma: Trakea cerrahisi anatomik ve teknik açıdan zor olsa da, benign lezyonlarda küratif olması, malign lezyonlarda ise ek radyoterapi ile başarılı sonuçlar alınması nedeniyle çok önemlidir. Uygun merkezlerde, deneyimli ekipler tarafından yapılan trakea cerrahisinin sonuçları oldukça başarılıdır.
