The effect of physical properties on postoperative bleeding in coronary artery bypass surgery

Abstract

Background: In the early postoperative period of open heart surgery, most common complication requiring 3-5% of exploration appears as bleeding. Bleeding in the early postoperative period is important risk factor in terms of morbidity and mortality. In this study, we investigated the effects of sex and physical properties on postoperative bleeding. Methods: Of 525 patients undergoing coronary artery bypass grafting, 209 were randomly divided into two groups according to the amount of bleeding (group I ≥750 ml/24 h, group II <750 ml/24 h) and were retrospectively evaluated for physical characteristics, preoperative, operative, and postoperative parameters.Results: At the end of the first day, drainage was less than 750 ml (mean 459±131.8 ml) in 174 patients (83.3%), and 750 ml or above (mean 948±188.3 ml) in 30 patients (16.6%) (p=0.0001). Bleeding indices were 528±102.7 ml/m2 and 266.0±77.7 ml/m.2 in group I and II respectively (p=0.0001). In group I, there were five females (16.6%) and 25 males (83.3%). In group II, there were 95 females (54.5%) and 79 males (45%), the number of females in group II was significantly higher (p<0.02). There were significant differences between the two groups with respect to the mean height (1.64±0.08 m in group I, 1.59±0.09 m in group II; p<0.005) and body mass index (27.1±4.0 kg/m.2 in group I, 28.6±4.1 kg/m.2 in group II; p=0.04).Conclusion: Severe postoperative hemorrhage is associated with significant morbidity and mortality. Preoperative risk stratification may help identify subgroups of patients who has high risk for bleeding after coronary artery bypass grafting. Further studies are needed to understand the factors relevant to less hemorrhage in patients with short height and in female subjects.

Amaç: Açık kalp cerrahisi sonrası erken dönemde %3-5 oranında eksplorasyon gerektiren en sık komplikasyon kanamadır. Kan ve ürünlerinin kullanımı, transfüzyon reaksiyonlarını, transfüzyon geçişli enfeksiyonları ve maliyet artışını birlikte getirmektedir. Bu çalışma, ameliyat sonrası kanama üzerine cinsiyet ve fiziksel özelliklerin etkilerini belirlemek için planlandı. Çalışma planı: İzole koroner bypass uygulanan 525 hasta arasından rasgele seçilen 209 hasta kanama miktarlarına göre iki gruba ayrılarak (grup I ≥750 ml/24 saat, grup II <750 ml/24 saat) demografik, ameliyat öncesi laboratuvar verileri, amaliyat ve erken ameliyat sonrası bulgular açısından retrospektif olarak incelendi.Bulgular: Birinci gün sonunda 174 hastada (%83.3) 750 ml'nin altında (ort. 459±131.8 ml), 30 hastada (%16.6) 750 ml ve üzerinde (ort. 948±188.3 ml) drenaj saptandı (p=0.0001). Kanama indeksleri grup I ve grup II için sırasıyla 528.0±102.7 ml/m2 ve 266.0±77.7 ml/m.2 bulundu (p=0.0001). Grup I'deki 30 hastanın beşi (%16.6) kadın, 25'i erkek (%83.3), grup II'deki 174 hastanın 95'i (%54.5) kadın, 79'u erkek (%45) idi. Grup II'deki kadın cinsiyet sıklığı istatistiksel olarak anlamlı bulundu (p<0.02). İki grup arasında boy uzunluğu açısından da anlamlı fark görüldü (grup I'de ort. 1.64±0.08 m, grup II'de ort. 1.59±0.09 m; p<0.005). Vücut kütle indeksleri grup I ve grup II için sırasıyla 27.1±4.0 kg/m.2 ve 28.6±4.1 kg/m.2 bulundu, bu fark istatistiksel olarak anlamlıydı (p=0.04).Sonuç: Koroner arter cerrahisini takiben gelişen kanama komplikasyonları morbidite ve mortalite artışına yol açmaktadır. Kanama için risk faktörlerinin ameliyat öncesinde belirlenmesi bu sorunların çözülmesine yardımcı olacaktır. Kısa boylu ve kadın hastalarda kanama azlığı ile ilgili faktörlerin belirlenmesine yönelik çalışmalara gerek duyulmaktadır.
