The role of mediastinoscopy in the staging of lung cancer

Abstract
In this study, patients who were diagnosed as lung cancer and underwent mediastinoscopy for lung cancer staging were reviewed retrospectively. and the role of medastinoscopy in staging of tumor was evaluated.Methods: Between September 2001 and May 2006, 95 patients (64 males, 31 females) underwent standard cervical mediastinoscopy for lung cancer staging in our clinic.Results: Fifty-two patients (%54) were staged as negative for mediastinal lymph nodes, 35 patients (%36) as N2, eight patients (%10) as N3. Patients who were not diagnosed as N2 or N3 disease with mediastinoscopy underwent pulmonary resection and mediastinal lymph node dissection via thoracotomy.Conclusion: Although mediastinoscopy is an invasive procedure, it is highly accurate and easy to perform with minimal mortality and morbidity rates, therefore, it is still the gold standard for lung cancer staging.

Amaç: Bu çalışmada; akciğer kanseri tanısı konan ve mediastinoskopi yapılan olgular geriye dönük olarak incelendi ve mediastinoskopinin tümör evrelemesindeki etkinliği değerlendirildi.Çalışma planı: Eylül 2001-Mayıs 2006 tarihleri arasında akciğer kanseri evrelemesi amacıyla 95 olguya (64 erkek, 31 kadın) standart servikal mediastinoskopi uygulandı.Bulgular: Elli iki olguda (%54) örnek alınan lenf bezlerinde tümör negatif, 35 olguda (%36) N2, sekiz olguda (%10) N3 olarak tespit edildi. Mediastinoskopi ile N2 veya N3 olarak tespit edilmeyen hastalara torakotomi ile anatomik rezeksiyon uygulandı ve mediastinal lenf nodu disseksiyonu yapıldı.Sonuç: Mediastinoskopi, invaziv olmasına karşın, doğruluk oranı yüksek, uygulaması kolay, morbidite ve mortalite oranlarının minimal olması nedeniyle, akciğer kanseri evrelemesinde halen altın standarttır.
