The effect of hormone replacement therapy on early results of coronary artery bypass surgery in postmenopausal women

Abstract
Methods: The study included 47 postmenopausal women (mean age 59±3 years; range 57 to 63 years) who underwent coronary artery bypass surgery between 2002 and 2006. The patients were evaluated in two groups: those who were not receiving HRT, and those who were under HRT. The preoperative, operative, and postoperative data of the patients were evaluated.Results: The perioperative need for blood and blood products was significantly less in patients receiving HRT (p<0.05). Patients under HRT had a higher incidence of postoperative atrial fibrillation (p<0.05). During intensive care stay and service follow-up, body temperatures were significantly lower in the HRT group (p<0.05). This was especially notable on the postoperative third and seventh days. Although blood cell counts in terms of hematocrit and hemoglobin levels were similar in two groups, patients who were not receiving HRT exhibited significantly higher leukocyte levels, especially after the third postoperative day (p<0.05).Conclusion: Hormone replacement therapy does not seem to have an effect on early mortality in women after coronary bypass surgery; however, it may decrease postoperative morbidity which is higher in women.

Amaç: Menopoz sonrası dönemde hormon replasman tedavisinin (HRT) koroner arter bypass ameliyatı, ameliyat sonrasındaki hastane içi mortalite ve morbiditeleri üzerindeki etkileri araştırıldı. Çalışma planı: Çalışmaya, 2002-2006 yılları arasında menopoz sonrası dönemde, koroner arter hastalığı nedeniyle aorta koroner greft bypass ameliyatı yapılan 47 hasta (ort. yaş 59±3; dağılım 57-63) alındı. Hastalar HRT kullanıp kullanmama temelinde iki gruba ayrıldı. Tüm olguların demografik ve ameliyat öncesi ve sonrası bulguları kaydedildi.Bulgular: Hormon replasman tedavisi gören hastalarda ameliyat sırasında ve sonrasında kan ve kan ürünlerinin kullanımı anlamlı derecede daha az bulundu (p<0.05). Yoğun bakım ve servis takipleri süresince, bu grupta atriyal fibrilasyon sıklığı daha yüksek (p<0.05) bulunmasına karşın, vücut sıcaklığı anlamlı derecede daha düşük seyretti (p<0.05). Bu fark, özellikle ameliyat sonrası üçüncü ve yedinci günlerde belirgindi. Tam kan sayımlarında hematokrit ve hemoglobin düzeyleri açısından iki grup arasında fark gözlenmese de, HRT kullanmayan grupta lökosit değerleri, özellikle ameliyat sonrası üçüncü günden itibaren daha yüksek bulundu (p<0.05).Sonuç: Hormon replasman tedavisinin, koroner cerrahisi sonrası kadınlarda erken dönem mortalite üzerine etkisi olmamakla birlikte, kadınlarda daha yüksek olan ameliyat sonrası morbiditeyi azaltıcı etkisi olabilir.
