Hamartoma of the lung: An analysis of 15 cases

Abstract
Background: We retrospectively aimed to evaluate the clinicopathological features of pulmonary hamartomas for which surgical resections were performed. Methods: From January 2000 to December 2006, 15 patients (11 males, 4 females; mean age 53.5 years; range 42 to 68 years) with pulmonary hamartomas underwent surgical resection in our department. In this study, these cases were evaluated retrospectively with respect to clinical, histopathological and radiological features, surgical methods and the results of treatment. Results: Fifteen cases with pulmonary hamartomas were included in the study. Six of these 15 patients were asymptomatic (40%); in the remainder cases chest pain, cough, shorthness of breath, recurrent pulmonary infections or hemoptysis were present. There was calcification in six patients (40%). In 11 cases the mass was <3 cm in diameter (73.3%) and in nine cases it was located in the superficial edge of the lung, infour cases it had endobronchial location. The surgical procedures which were performed were enuclation in two cases, bronchotomy in two, wedge resection in seven, segmentectomy in one, lobectomy in one and pneumonectomy in two. No deaths ocurred during operations and there were only minor postoperative complications. There was no postoperative death and no recurrent pulmonary hamartomas developed in the 12-month mean follow up period (4 to 55 months). Conclusion: Pulmonary hamartomas usually present as solitary pulmonary nodules. Definite diagnosis and the treatment can be achieved by surgical resection with minimal morbidity.

Amaç: Bu çalışmada cerrahi rezeksiyon uyguladığımız pulmoner hamartom olguları geriye dönük olarak değerlendirildi. Çalışma planı: Ocak 2000 ile Aralık 2006 tarihleri arasında kliniğimizde 15 pulmoner hamartom olgusuna (11 erkek, 4 kadın; ort. yaş 53.5 yıl; dağılım 42-68 yıl) cerrahi rezeksiyon yapıldı. Çalışmamızda bu olgular; klinik, histopatolojik ve radyolojik özellikleri, uygulanan cerrahi işlemler ve tedavi sonuçları açısından geriye dönük olarak değerlendirildi. Bulgular: Çalışmaya dahil edilen pulmoner hamartomlu 15 hastanın altısı (%40) asemptomatik iken, geriye kalanlarda göğüs ağrısı, öksürük, nefes darlığı, tekrarlayan pulmoner enfeksiyonlar ya da hemoptizi yakınmaları vardı. Altı hastada (%40) kalsifikasyon gözlendi. Olguların 11’inde lezyonun kitle çapı 3 cm’den küçük bulunurken (%73.3); lezyonlar olguların dokuzunda akciğer periferinde, dördünde endobronşiyal yerleşimli idi. Uygulanan cerrahi yöntemlere bakıldığında ise; olguların ikisine bronkotomi, ikisine enükleasyon, yedisine wedge rezeksiyon, birine segmentektomi, birine lobektomi ve ikisine da pnömonektomi yapıldığı görüldü. Ameliyat sırasında ölüm olmadı, ameliyat sonrası dönemde ise birkaç minör komplikasyon görüldü. Ameliyat sonrası dönemde ölüm ve ortalama 12 aylık takip süresince (4-55 ay) yineleme görülmedi. Sonuç: Pulmoner hamartomlar sıklıkla soliter pulmoner nodüller şeklinde ortaya çıkarlar. Kesin tanı ve tedavi, minimal morbiditeye sahip cerrahi rezeksiyonla başarılı bir şekilde yapılabilir.
