The effect of application of cyclosporin and tacrolimus (FK506) on newly formed atherosclerosis and lipid profile in rats

Abstract
Background: The aim of this study was to analyse and compare the effects of cyclosporin and tacrolimus (FK506) on de novo atherosclerosis in Apo E-/- rats.Methods: These experimental Apo E-/- rats were separated into three groups. Group 1 (control group; n=6) rats were fed 0.5 cc/day olive oil, group 2 rats (n=4) were treated with 10 mg/kg/day cyclosporin in a 0.5 cc olive oil diet for four weeks and group 3 rats (n=5) were treated with 0.05 mg/kg/day tacrolimus in a 0.5 cc olive oil diet for four weeks. At the end of the 4th week, morphological analyses were made with light microscope hematoxylin-eosine x100 and x400. Levels of total cholesterol, HDL cholesterol and triglycerides in these three groups were compared in blood samples obtained from the aorta of the rats.Results: There was no statistical difference with respect to total cholesterol and HDL cholesterol levels between the groups. Triglyceride level was considerably higher in cyclosporin and tacrolimus group than in control group, but there was no statistical difference in both cyclosporin and tacrolimus groups. In the morphological analysis with light microscope, there was no difference with respect to widening, thickening, intima-media proportion, aterom plaque morphology, endothelial desquamation, deposition of sub-endothelial lipid, degeneration in elastic ligaments of media layers, abnormalities in endothelial cell composition in the intima and media layers of the common carotid artery. Partial intimal thickness was detected in one of the cyclosporin group sample.Conclusion: There was intimal thickening in one of the subjects in cyclosporin group, while there was no significant difference in cholesterol level. This suggests that cyclosporin may potentiate atherosclerosis by stimulation of vascular smooth muscle cell proliferation independent of its hyperlipidemic effect.

Amaç: Bu çalışmada, siklosporin ve takrolimusun (FK506) Apo E-/- sıçanlarda yeni oluşan de novo ateroskleroz üzerine olan etkisi, karşılaştırmalı olarak incelendi.Çalışma planı: Çalışmada kullanılacak olan Apo E-/- sıçanlar üç gruba ayrıldı; grup 1'deki sıçanlara (kontrol grubu; n=6) dört hafta süresince günlük 0.5 cc zeytinyağı, grup 2'deki (n=4) sıçanlara dört hafta süresince günlük 0.5 cc zeytinyağıyla birlikte 10 mg/kg/gün siklosporin ve grup 3'teki (n=5) sıçanlara dört hafta süresince günlük 0.5 cc zeytinyağıyla birlikte 0.05 mg/kg/ gün takrolimus oral yolla uygulandı. Dördüncü haftanın sonunda; sıçanların hematoksilen-eozin x100 ve x400 büyütmedeki ışık mikroskopisi incelemeleri yapıldı. Sıçanların aortundan alınan kan örneğinden her üç grubun total kolesterol, trigliserit ve HDL kolesterol değerleri karşılaştırıldı.Bulgular: Gruplar arasında total kolesterol ve HDL kolesterol medyanları arasında anlamlı bir fark bulunmadı. Siklosporin ve takrolimus gruplarındaki trigliserit değerinin kontrol grubuna göre anlamlı derecede yüksek olduğu fakat her iki grup arasında istatistiksel bir fark olmadığı görüldü. Işık mikroskopisi incelemelerinde; sıçanların ortak karotis arterlerinin intima ve media tabakasında genişleme, kalınlaşma, intima media oranı, aterom plak morfolojisi, endotelyal deskuamasyon, subendotelyal lipit birikimi, media tabakasındaki elastik lif yapılarında dejenerasyon, endotelyal hücre diziliminde düzensizlik yönünden gruplar arasında anlamlı bir fark tespit edilemedi. Siklosporin grubundan bir örnekte, parsiyel intimal kalınlaşma tespit edildi.Sonuç: Siklosporin grubundan bir örnekte kolesterol düzeyinde anlamlı fark yok iken, intimal kalınlaşmanın tespit edilmesi; siklosporinin, hiperlipidemik etkisinden bağımsız olarak, doğrudan vasküler düz kas proliferasyonunu artırıcı etkisiyle aterosklerozu hızlandırabileceği düşüncesini akla getirdi.
