Diaphragmatic eventration becoming symptomatic after trauma

Abstract

Diaphragmatic eventration is the abnormal elevation of the diaphragm as a result of paralysis or atrophia of the musculer fibers. It is generally asymptomatic and detected accidently. It rarely requires emergency surgery. An 80-year-old male patient, who had a previous laparotomy 12 years ago, admitted to our clinic with complaints of severe dyspnea and ortopnea that developed after severe trauma. The patient urgently underwent surgery and diaphragm plication was performed. Patients with diaphragmatic eventration may experience severe respiratory distress after traumas that may occur during their follow-ups. It's treatment is the surgical correction of diaphragm.

Diyafram evantrasyonu, kas fibrillerinin atrofisi veya paralizisi sonucu gelişen anormal diyafram elevasyonudur. Sıklıkla asemptomatik seyreder ve tesadüfen tespit edilir. Çok nadiren acil cerrahi tedavi gerekir. On iki yıl önce ileus nedeniyle laparatomi geçirmiş olan 80 yaşında erkek hasta, şiddetli travma sonrası gelişen şiddetli dispne ve ortopne yakınması ile kliniğimize başvurdu. Hasta hemen ameliyata alınarak diyaframa plikasyon uygulandı. Diyaframatik evantrasyonlu hastaların takibi esnasında oluşabilecek travmalarda, hastada ciddi solunum sıkıntısı gelişebilir. Tedavisi diyaframın cerrahi olarak düzeltilmesidir.
