Planning o f thoracic surgical spec ialists in light of scie ntific data in Turkey: what is the number of thoracic surgical spec ialists and w hat should it be?

Abstract
Background: This study aims to investigate the adequate numberof thoracic surgical specialists in Turkey in light of scientific data.Methods: In this study, work force (manpower) particular tothoracic surgery was calculated by using data including numberof beds, hospitalization rate, bed occupancy rate, and meanduration of hospitalization in our country, as well as TurkishStatistical Institute, European Union and United States ofAmerica data compared to targets, disease burden, population,and countries with high human development index. Also,workload was calculated with projection method. With thisprojection, workload components and European standards werecompared and coefficients were determined.Results: According to calculations over thoracic surgerydepartment number of beds (n=1052), hospitalization rate(5%), bed occupancy rate (80%), and mean hospitalizationduration (7 days and less), the number of required thoracicsurgical specialists in our country was 344. Since, accordingto Turkey Burden of Disease Study (2004), thoracic surgeryspecific disease burden was 0.68% of total disease burden,0.68% of total specialized physicians should be thoracicsurgeons, and this rate equals to the number of 475. In Turkey,since one thoracic surgeon corresponded to 185,000 peoplein 2007, according to 2013 population, number of thoracicsurgeons should have been 412. Numbers of required thoracicsurgeons according to European Union and United States ofAmerica data were 421 and 196, respectively. According toworkload analysis performed by projection, 89% of currentthoracic surgeons, in other words, 482 are sufficient.Conclusion: According to the results of our study, number ofthoracic surgical specialists in Turkey is very high. If risingnumber of thoracic surgeons is not prevented over the years, itmay be inevitable for some thoracic surgeons to be engaged inother jobs in near future.

Amaç: Bu çalışmada bilimsel veriler ışığında Türkiyede uygungöğüs cerrahisi uzmanı sayısı araştırıldı.Çalışmaplanı: Bu çalışmada ülkemizdeki yatak sayısı,hastanede yatış oranı, yatak doluluk oranı, ortalama yatışsüresi verileri ile beraber Türkiye İstatistik Kurumu, AvrupaBirliği ve Amerika Birleşik Devletleri verileri kullanılarakhedeflere, hastalık yüküne, nüfusa ve insani gelişmişlikendeksi yüksek olan ülkelere göre göğüs cerrahisineözgü işgücü (insan gücü) hesaplandı. Ayrıca projeksiyonyöntemi ile iş yükü hesaplandı. Bu projeksiyon ile işyükü bileşenleri ve Avrupa standartları karşılaştırılarakkatsayılar belirlendi.Bul gu lar: Göğüs cerrahisi bölümü yatak sayısı (n=1052),hastanede yatış oranı (%5), yatak doluluk oranı (%80)ve ortalama yatış süresi (7 gün ve daha az) üzerindenyapılan hesaplamalara göre ülkemizde gereken göğüscerrahisi uzmanı sayısı 344 idi. Türkiye Hastalık YüküÇalışmasına (2004) göre ülkemizde göğüs cerrahisine özgühastalık yükü toplam hastalık yükünün %0.68i olduğunagöre, toplam uzman hekimlerin %0.68i göğüs cerrahıolmalıdır ve bu oran 475 sayısına denktir. Türkiyede 2007yılında 185.000 kişiye bir göğüs cerrahı düştüğünden, 2013yılı nüfusuna göre göğüs cerrahı sayısı 412 olmalı idi.Ülkemizde 2013 için gerekli göğüs cerrahı sayısı AvrupaBirliği ve Amerika Birleşik Devletleri verilerine göresırası ile 421 ve 196 idi. Projeksiyon ile yapılan iş yüküanalizine göre, mevcut göğüs cerrahlarının %89u, yani482si yeterlidir.Sonuç: Çalışma sonuçlarımıza göre, Türkiyedeki göğüscerrahisi uzmanı sayısı çok yüksektir. Yıllar geçtikçe göğüscerrahı sayısının yükselişi engellenmez ise yakın gelecektebazı göğüs cerrahlarının başka işler ile uğraşması kaçınılmazolabilir.
