A rare association of Poland’s syndrome and pneumothorax

Abstract
Poland’s syndrome is a rare congenital anomaly, characterized by a partial or complete absence of pectoralis major muscle, absence of minor pectoral muscles, absence or hypoplasia of breasts and/or nipples, absence of ribs in a varying degree, hand abnormalities, and hypoplasia of the subcutaneous tissue. However, its coexistence with pneumothorax is very rare. A 51-year- old male patient was admitted to our emergency department with complaints of progressive chest pain and dyspnea which began a week ago. Physical examination revealed that breath sounds decreased on the right side with an absence of pectoralis major muscle. Plain chest X-ray showed total pneumothorax on the right side. The patient underwent bullectomy and pleural abrasion using video-assisted thoracoscopic surgery. We present this case to draw attention to the rare association of Poland’s syndrome with spontaneous pneumothorax.

Poland sendromu nadir gözlenen doğuştan bir anomalidir ve majör pektoral kasın kısmen veya tamamen yokluğu, minör pektoral kasın yokluğu, meme veya meme başının yokluğu veya hipoplazisi, kaburgaların değişen oranlarda yokluğu ve el anomalileri, cilt altı yağ dokusunun hipoplazisi ile karakterizedir. Ancak pnömotoraks ile birlikteliği çok nadirdir. Elli bir yaşında erkek hasta acil polikliniğimize bir haftadır var olan ve gittikçe artan göğüs ağrısı ve nefes darlığı yakınması ile başvurdu. Yapılan fizik muayenesinde sağ tarafta solunum seslerinde azalma ve sağ pektoralis majör kas yokluğu saptandı. Direkt akciğer grafisinde sağ hemitoraksta total pnömotoraks izlendi. Hastaya video yardımlı torakoskopik cerrahi ile büllektomi ve plevral abrazyon uygulandı. Bu olgu Poland sendromu ve spontan pnömotoraksın nadir birlikteliğine dikkat çekmek amacıyla sunuldu.
