Evaluation of the effects of obesity and polycystic ovary syndrome on glucose metabolism and sex hormone levels

Abstract

Aim: The existence of insulin resistance in women with polycystic ovary syndrome has been generally accepted, but, the debate is still continuing about whether this is a part of syndrome or is the result of obesity that is usually seen in this patient group. In this study, we aimed to investigate the effect of obesity on sex hormone levels and glucose metabolism in women with and without polycystic ovary syndrome. Institution: Mersin University, School of Medicine, Department of Obstetrics and Gynecology Materials and Methods: Sixty women with polycystic ovary syndrome and 60 age-matched controls without any endocrinopathy were included in the study. The patients with a body mass index 28 kg/mr and over were regarded to be obese. Insulin levels, glucose tolerance and sex hormone levels were evaluated in these patients. Results: Insulin levels were found higher in women with polycystic ovary syndrome regardless of obesity. Insulin level was higher only in obese patients in the control group. Regardless of obesity; testosterone, androstenedione and dehydroepiandrosterone-sulfate levels were higher than the control group. The sex hormone levels were within the normal range, although the obese control patients had hyperinsulinemia. Conclusion: In our study, hyperandrogenemia with both adrenal and ovarian origin had been found in patients with polycystic ovary syndrome. However, this could not be explained with obesity or hyperinsu linemia, alone. Certainly, hyperinsulinemi in patients with polycystic ovary syndrome contribute to androgen production, but, different mechanisms should also be investigated.

Özet

Amaç: Polikistik over sendromlu kadınlarda periferik insülin direnci varlığı genel olarak kabul görmektedir ancak bunun sendromun bir parçası mı olduğu, yoksa bu hasta grubunda sık gözlenen obesiteden mi kaynaklandığı halen tartışma konusudur. Bu çalışmada, obesitenin, polikistik over sendromu olan veya olmayan hasta gruplarında seks hormon düzeyleri ve glikoz metabolizması üzerindeki etkisini araştırmayı amaçladık. Çalışmanın Yapıldığı Yer: Mersin Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı Materyal ve Metod: 60 polikistik over sendromlu hasta ve yaş açısından eşleştirilmiş 60 düzenli adet gören ve herhangi bir endokrinopatisi olmayan kadın çalışmaya dahil edildi. Vücut kitle indeksi 28 kg/m2 ve üzerinde olan hastalar obez kabul edildi. Bu hastalarda insülin düzeyleri, glukoz toleransı ve seks hormon düzeyleri değerlendirildi. Bulgular: Polikistik over sendromu olan kadınlarda obesiteden bağımsız olarak insülin düzeyleri yüksek bulundu. Kontrol grubunda ise sadece obez hastalarda insülin düzeyi yüksekti. Polikistik over sendromlu hastalarda testosteron, androstenedion ve dehidroepiandrosteron-sülfat düzeyleri yine obesiteden bağımsız olarak kontrol grubundan yüksekti. Obez kontrol hastalarında hiperinsülinemi olmasına rağmen seks hormon düzeyleri normal sınırlarda idi. Sonuçlar:Çalışmamızda, PKOS hastalarında hem adrenal hem de over kaynaklı hiperandrojenemi saptandı. Ancak bu durum sadece obesite veya sadece hiperinsülinemi ile açklanamamaktadır. Elbette PKOS hastalarında hiperinsülineminin, androjen üretimine katkısı vardır ancak daha farklı mekanizmaların da araştırılmaı gerekir.
