Results of preimplantation genetic diagnosis in embryos obtained from the cases with Klinefelter's syndrome

Abstract
Objective: With the application of p re implantation genetic diagnoses (PGD), possible genetic contribution of spermatozoa obtained from 47,XXY Klinefelter patients on preimplantation embryos was analyzed in 8 couples. Institution:Istanbul Memorial Hospital, ART, Reproductive Endocrinology ScGenetics Centre, Istanbul. Materials and Methods: 8 ART cycles of 8 couples in which male partner was diagnosed as Klinefelter Syndrome were retrospectively analyzed. Embryo biopsy for preimplantation genetic diagnosis (PGD) was performed on day 3 of embryo development. Blastomeres were then analyzed by fluorescent in situ hybridization (FISH) for chromosomes 13, 18, 21 X and Y. Embryo transfer was performed with chromosomally normal embryos obtained after PGD. Results: Interpretable FISH results were obtained for 28 out of 33 embryos biopsied (84.8%). 23 blastomeres were analyzed for chromosomes 13,18,21, X, Y.9 out of 23 embryos were diagnosed as abnormal (39.1%). 5 out of 9 contained sex chromosome abnormalities (55.5%). Two were diagnosed as 47,XXY and three were found to contain monosomy X. Besides sex chromosomal abnormalities, other abnormalities detected were haploidy, triploidy, monosomy 13, monosomy 18 and trisomy 13. 5 blastomeres were analyzed for sex chromosomes only and all of them were found to be normal. Overall, the rate of sex chromosome abnormality in biopsied embryos was found to be 17,8% (5/28). Moreover, among 33 embryos biopsied, 5 of the 8 zygotes, which were classified as a poor prognosis group according to pronuclear morphology scoring, showed as impaired growth profile after biopsy and were found to be chromosomally abnormal. Elimination of abnormal embyos and transfer of normal ones resulted in three pregnancies in 8 cycles. One was delivered to term, 2 are ongoing. Conclusions: Our results show that there is infant elevated chromosomal abnormality for both sex chromosomes and autosomes in embryos developed from Klinefelter males. Also together with PGD, embryo scoring according to pronuclear morphology can give additional benefit for selecting chromosomally abnormal embryos. Therefore, PGD 'should be highly recommended and this information should be discussed with the couple when genetic counseling is given.

Amaç: Bu çalışmada mozaik olmayan 47,XXY karyotip yapısına sahip Klinefelter Sendromlu olgularda preimplantasyon genetik tanı tekniği ile embriyolardaki kromozomal anomali oranları araştırılmıştır. Çalışmanın Yapıldığı Yer: İstanbul Memorial Hastanesi, Yardımcı Üreme Teknikleri ve Genetik Tanı Merkezi, İstanbul Materyal ve Metod: Çalışmada İstanbul Memorial Hastanesi YÜT, Reprodüktif Endokrinoloji ve Genetik Merkezine başvuran ve yardımcı üreme teknikleri (YÜT) programına alınan 8 Klinefelter Sendromu tanısı konmuş çifte ait sikluslar retrospektif olarak incelenmiştir. ICSI sonrası elde edilen embriyolarda, embriyo gelişiminin 3.günü uygun gelişim düzeyinde olanlarına preimplantasyon genetik tanı (PGT) amaçlı biyopsi işlemi uygulanmıştır. Biyopsi sonrası alınan blastomerler floresan in situ hibridizasyon (FISH) tekniği kullanılarak 13., 18., 21., X ve Y kromozomları yönünden incelenmiştir. FISH sonucu normal olarak değerlendirilen embriolar transfer edilmiştir. Bulgular: PGT yöntemleri ile 8 çiftte ait toplam 33 embriyoda biyopsi işlemi uygulandı. Biyopsisi yapılan 33 embriyonun 28’ inden (%84.8) değerlendirilebilir FISH sonuçları elde edildi. 23 blastomer 13,18, 21, X ve Y kromozomları için analiz edildi ve 9’ u anormal olarak değerlendirildi (%39.8). 5 embriyoda cinsiyet kromozomu anomalisi gözlendi (%55.5). 2 tanesi 47,XXY ve 3 tanesi monozomi X olarak değerlendirildi. Cinsiyet kromozomu anomalileri dışında, ayrıca haploidi, triploidi, monozomi 13, monozomi 18, trizomi 13 anomalileri belirlenen diyer anomalileri oluşturmaktaydı. Değerlendirilebilir sonuç alınan diğer 5 embriyoya ait blastomerler sadece cinsiyet kromozomları için analiz edildi ve hepsi normal bulundu. Biopsi yapılan embriyoların toplamında cinsiyet kromozomu anomalisi oranı %17.8 (5/28) olarak bulundu. Ayrıca, biopsi yapılan 33 embriyo arasında 8 zigottan, pronüklear morfoloji skoruna göre zayıf prognoz olarak sınıflandırılan 5’i biopsiden sonra kötü gelişim gösterdi ve kromozomal olarak anormal oldukları saptandı. Anormal embriyoların elenmesi ve normal olanlarla transfer yapılması sonucunda 8 siklusta 4 gebelik (%50) elde edildi. İkisi tekil, diğer ikisi ikiz olmak üzere, elde edilen dört gebelik te sağlıklı doğumla sonuçlandı. Sonuçlar:Bu sonuçlar, Klinefelter erkeklerden gelişen embriyolarda hem cinsiyet kromozomlarında, hem de otozomlarda yüksek oranda anomali olduğunu göstermiştir. Ayrıca, PGT’ ye ilave olarak pronüklear morfolojiye göre embriyo skorlamasının kullanımı kromozomal olarak normal olan embriyoların seçiminde fayda sağlamıştır. Bu nedenle, Klinefelter Sendromu olan vakalarda PGT önerilmeli ve bu bilgi genetik danışmanlık sırasında çiftlere detaylı olarak anlatılmalıdır.
